摘要
痰隐血Ⅰ期试验在1011例门诊可疑肺癌、无血痰的标本中,作隐血检查,在604例痰隐血阴性标本中发现24例癌,检出率为3.97％;在407例痰隐血阳性标本中发现80例癌,检出率为19.65％。后者比前者高5倍。Ⅱ期试验从1988年至1990年8月底,对40岁以上职工(有少数40岁以下者主动要求参加检查),作痰隐血初筛普查,共14431人(送痰1次9888人,2次者4543人),隐血阳性反应有1942人(占13.5％),每人涂片2张,巴氏染色,镜检发现31例癌(中晚期25例,早期6例),检出率为1.6％(31/1942),痰隐血阴性组癌检出率为0.04％(5/12489),前者比后者提高39.9倍,显示该方法有初筛高危险人群价值。方法简单、经济,易进入家庭,由个人定期自检,发现痰有隐血反应时,应送痰去医院作细胞学镜检。
Stage I test for sputum occult blood test in outpatients with suspected lung cancer, total number of 1011 patients, among 604 patients of sputum were negative of occult blood test which detected 24 patients with lung cancer, false negative rate of 3.97% (24/604). 407 patients were positive reaction which detected 80 patients with lung cancer, positive rate of 19.65% (80/407) . The positive rate is high 5 fold than negative group of sputum of occult blood test From 1988 to 1990, stage Ⅱ test were finished in The Capital Steel works, The Tin Mines, The Changchun First Motorcar Manufactory,and Tangshan City over forty year old workers and cardres were asked to send sputum for ond or two times for ocuclut blood test, among 9888 persons sent on times of sputum, 4543 persons for two times, but saliva example were extruded. Effective number were 14431 persons, among 1942 persons of sputum were positive response to occult blood test (13.5%). Which sputum were made two slice glass, stained with Papanicolaou's, 31 cases with cancer cells were found under microscope(25 mild-later stage, 6 early cases). Detected rate was 1.6% (31/1942), it is high 34.8 folds than morbidly of lung cancer. It shows that the method for primary screening lung cancer is worth, cheap and it can be used by himself periodical. If sputum is positive reaction to occult blood test, who should send sputum to hospital for cytological examination. It can be detected early lesion, such as hyperplasia or displasia and cancer cells.
