摘要
目的探讨非典型性脑膜瘤(atypical meningioma,AM)的临床病理学特点和复发率。方法回顾性分析16例AM的临床资料、影像学、病理形态学和免疫表型特点,并随访患者的复发率和生存情况。结果 16例AM中男性6例,女性10例,发病年龄24～77岁,中位数57岁。多发于大脑凸面、大脑镰和矢状窦,以头痛为主要临床症状。组织学表现为核分裂活性增高、平均核分裂象>5个/10 HPF、细胞密度高,细胞核具有一定的异型性,核质比增高,核仁明显,局部伴有坏死。免疫表型:Ki-67增殖指数平均为14.3%,vimentin阳性,EMA以阳性表达为主。15例获随访结果,7例复发,2例复发患者再次手术后死亡,复发率43.8%。结论 AM是一种高复发性的脑膜瘤,其核分裂活性高,细胞密集,局部伴有坏死,Ki-67增殖指数为14.3%。
Purpose To investigate the clinicopathological characteristics and recurrence rate of atypical meningioma （AM）. Methods The clinicopathologic data, MRI, morphological features and immnnohistochemical characteristics were reviewed in 16 cases of AM. The survival and recurrence were observed. Results There were 6 males and 10 females. The age range was from 24 -77 years with mean of 57 years. The tumor most commonly occurred in facies convexa cerebri, falx cerebri and longitudinal sinus, and headache presented as a main symptom. Microscopically, AM was characterized by high mitotic activity （5/10 HPF） , hypercellularity, high ra- tio of nucleus/cytoplasm, and prominent nucleoli, and accompanied by localized necrosis. The mean Ki-67 proliferation index was 14. 3%. All the cases showed positive staining for vimintin and most of cases expressed EMA （14/16）. Fifteen cases got follow up da- ta, showing that recurrence was in 7 cases, and two died 3 years after operation, and recurrence rate was 43.8%. Conclusion AM is a highly recurrent meningoma with high mitotic activity, hypercellularity, focal necrosis and high proliferation index.
