经尿道单极与双极前列腺电切术治疗高龄、高危良性前列腺增生的疗效对比 被引量：11

摘要 目的探讨经尿道前列腺电切术(TURP)与经尿道等离子体双极前列腺电切术(PKRP)治疗高龄、高危良性前列腺增生(BPH)的有效性和安全性。方法回顾性分析该院2008年5月至2010年10月收治的高危、高龄BPH患者124例,分别采用TURP(单极组)和PKRP(双极组)治疗,比较两组患者手术前后国际前列腺症状评分(IPSS)、生活质量评分(QOL)、残余尿量(RUV)、最大尿流率(Qmax)、电解质、手术时间和术中失血量、手术并发症等指标的变化。结果两组患者年龄、前列腺体积、术前IPSS、QOL、RUV、Qmax等指标均无统计学差异(P>0.05)。两组患者治疗后IPSS、QOL、RUV、Qmax均明显改善,与治疗前比较差异显著(P<0.05),但两组间比较无统计学差异(P>0.05)。单极组术后血糖〔5.8±0.6)mmol/L〕较术前〔(8.2±1.3)mmol/L〕升高(P<0.05),而双极组手术前后血糖水平未见明显变化。单极组术中出血量〔(282±28.6)ml〕较双极组〔(220±14.8)ml〕高(P<0.05)。结论经尿道单极与双极前列腺电切术均是高龄、高危BPH的安全、有效的治疗方式,双极前列腺电切在控制术中出血、减少术中、术后失血方面有优势,且对机体血糖无明显影响。
