嗜酸性粒细胞计数在哮喘儿童支气管肺泡灌洗液中的变化及孟鲁司特对其调节作用 被引量:2

Changes of eosinophils in bronchoalveolar lavage fluid of asthma in children and the modulation effect of montelukast

摘要 目的:探讨嗜酸性粒细胞(eosinophils,EOS)计数在哮喘儿童支气管肺泡灌洗液(bronchoalveolar lavage fluid,BALF)的变化及孟鲁司特对其调节作用。方法:选择住院的中、重度支气管哮喘急性发作患儿42例,随机分为对照组(n=21)和治疗组(n=21)。对照组给予常规治疗,治疗组在对照组基础上口服孟鲁司特钠咀嚼片5mg,qd,治疗4周。比较2组治疗前后肺功能与BALF-EOS比例变化。结果:对照组FEV1、PEF、FEF25治疗前分别为(1.39±0.22)L,(2.89±0.44)L·min-1,(1.82±0.29)L·min-1;治疗后分别为(1.85±0.28)L、(3.80±0.52)L·min-1、(2.30±0.43)L·min-1,差异有极显著性(P<0.01);治疗组FEV1、PEF、FEF25治疗前分别为(1.41±0.23)L、(2.91±0.41)L·min-1、(1.84±0.31)L·min-1;治疗后分别为(2.24±0.31)L、(4.50±0.54)L·min-1、(2.83±0.45)L·min-1,差异有极显著性(P<0.01)。治疗前对照组和治疗组EOS绝对计数及EOS比例,2组比较差异无显著性(P>0.05);治疗后对照组和治疗组EOS绝对计数分别为:(26.8±3.7)×107/L和(22.8±2.9)×107/L,EOS比例分别为:(4.98±1.09)%和(3.98±0.76)%,2组比较差异有极显著性(P<0.01)。结论:儿童哮喘中BALF-EOS水平升高,孟鲁司特钠可减少肺内EOS浸润数量,改善肺功能。 OBJECTIVE To observe the changes of eosinophils in bronchoalveolar lavage fluid and pulmonary function of asthma in children before and after montelukasrs treatmen〈 METHODS Forty two children with acute exacerbation of bron chial asthma were randomly divided into treatment group (n = 21 ) and control group (n = 21 ). The control group was treated with conventional therapy, and the treatment group was given montelukast besides the conventional therapy. The pulmonary function and level of EOS were observed before and after 4 weeks therapy between the two groups. RESULTS In the control group ,FEV1, PEF and FEF25 were (1.39 ± 0. 22) L, (2. 89 ± 0. 44) L·min^-1, (1. 82 ± 0. 29) L·min^-1 before therapy, and (1.85± 0. 28)L, (3.80 ± 0. 52)L·min^-1, (2. 30 ± 0. 43) L. min i after therapy; while in the treatment group, FEV1, PEF and FEF25 were (1.41 ± 0. 23)L.(2. 91± 0. 41 ) L·min^-1 , (1.84 ± 0. 31 )L·min^-1 before therapy, and (2. 24 ± 0. 31 )L, (4. 5/) ± 0. 54)L·min^-1 , (2. 83 ±0. 45)L·min^-1 after therapy. There was significant difference before and after therapy. The abso lute counting and the differential count of EOS before therapy was no significant difference between the two groups (P〉0. 05). While the absolute counting of EOS after therapy was (26. 8 ± 3.7) 10^7/L and (22. 8 ± 2. 9)×10^7/L in control group and treat ment group respectively, and the differential count of EOS after therapy was (4. 98 ± 1.09)% and (3.98 ± 0. 76) % respective- ly. There was significant difference between the two groups (P〈0. (11). CONCLUSION The level of eosinophils in bronchoal veolar lavage fluid is high in asthma. Montelukast can decrease the level of eosinophils and significantly improve the pulmonary function of asthma in children.
