Cys-c、hs-CRP和尿微量白蛋白在糖尿病患者肾功能早期损伤中的诊断价值 被引量:31

摘要 目的探讨血清胱抑素C(Cys-c)、超敏C反应蛋白(hs-CRP)以及尿微量白蛋白(U-mAlb)在糖尿病患者早期肾功能损伤的诊断价值。方法根据糖化血红蛋白(HbAlc)测定值,将148名Ⅱ型糖尿病患者分为两组:HbA1c<8%为控制较好组(70例),HbA1c>8%为控制较差组(78例)。所有入选的Ⅱ型糖尿病患者血尿酸(Sur)、血尿素氮(BUN)和肌酐(SCr)检测均正常,尿蛋白阴性。同时随机选取120名健康体检组为对照组,检测患者U-mAlb、Cys-c和hs-CRP,并比较各组均值及检测阳性率。结果血糖控制较差组Cys-c、hs-CRP和U-mAlb均高于对照组和血糖控制较好组(P<0.01),血糖控制较好组的Cys-c、U-mAlb高于对照组(P<0.05),两组的hs-CRP比较无统计学意义(P>0.05)。血糖控制较差组的Cys-c、U-mAlb诊断阳性率高于血糖控制较好组(P<0.01),hs-CRP在两组间的差异无统计学意义(P>0.05)。结论 Cys-c、hs-CRP和U-mAlb联合检测在糖尿病早期肾损伤中具有较好的诊断功能,具有较高的临床应用价值。 Objective To investigate the diagnostic value of Cys-C,hs-CRP and U-mAlb in early renal injury of diabetes patients.Methods 148 cases of TypeⅡ diabetes patients were devided into 2 groups according to the measured glycated hemoglobin HbA1c,serum glucose well control group(70 cases,HbA1c8%),and serum glucose poor control group(78 cases,HbA1c8%).The concertration of Sur,BUN,SCr,urine protein of all the diabetes patient were normal.120 healthy people were randomly selected as control group.Detecting the concentration of U-mAlb,Cys-c,hs-CRP and comparing the means of every group.Results U-mAlb,Cys-c,hs-CRP of serum glucose poor control group were higher than that of glucose well control group and control group(P0.01).Cys-C and U-mAlb of glucose well control group were higher than the control group(P0.05),hs-CRP of the two group has no significant difference(P0.05).The diagnostic positive rate of U-mAlb and Cys-c of serum glucose poor control group was higher than serum glucose well control group.The difference of hs-CRP in this two groups has no statistically significance(P0.05).Conclusion Using Cys-c,hs-CRP,and U-mAlb together has a high clinical value and a good diagnostic capabilities to early renal injury of diabetes patients.
