摘要
对社区失能老人长期照护需求进行分析,结合漕河泾街道社区卫生服务中心的特点,初步探索并实践一套社区失能老人"三老联动"一体化长期照料服务模式,为失能老人提供养老、护老、终老的社区医疗服务保障,提高失能老人的生活和生命质量,降低医疗护理费用,减轻家庭负担。
Through analyzing the needs of disabled elderlies for a long -tem care, we primarily explored and had practiced a integrated service mode of long - term care for community disabled elderlies. This service mode can provide disabled elderlies with community medical services of old - age support and care, and attending upon a dying elderly, so as to improve the quality of life, reduce the fees of medical care, and relieve the burden of family.
