摘要
目的探讨自我效能理论在全膝关节置换术后康复训练中的应用效果。方法选择接受全膝关节置换的患者共81例,93膝,采用历史对照设计,2009年收治的患者36例、41膝(对照组)未予自我效能提升训练,2010年45例、52膝(观察组)采用自我效能提升训练。采用一般自我效能感量表(GSES)进行自我效能评估,采用特殊外科医院的膝关节评分(HSS)进行膝关节评分,采用视觉模拟评分(VAS)进行膝关节疼痛评分。患者治疗前及术后1、3个月进行GSES和HSS评定,于治疗前及术后24 h、1个月、3个月时进行VAS评定。结果两组患者GSES得分的组间差异有统计学意义(F组间=20.2,P<0.05),在时间序列上的差异无统计学意义(F时间=1.4,P>0.05),两组患者组间和时间序列的交互效应间无统计学意义(F交互=1.1,P>0.05)。两组患者VAS得分组间差异有统计学意义(F组间=11.2,P<0.05),在时间序列上的差异有统计学意义(F时间=5.3,P<0.01),两组患者组间和时间序列的交互效应间有统计学意义(F交互=1.7,P<0.05)。两组患者HISS评分组间差异有统计学意义(F组间=13.5,P<0.05),在时间序列上的差异有统计学意义(F时间=6.4,P<0.01),两组患者组间和时间序列的交互效应无统计学意义(F交互=1.3,P>0.05)。结论提升自我效能的训练对于全膝关节置换患者缓解术后疼痛、改善术后关节功能有一定作用。
Objective To evaluate the effects of self - efficacy theory on rehabilitation of patients with total knee re- placement （TKR）. Methods A total of 81 patients （93 knees） received TKR. Historical control design was used. 36 （41 knees） admitted from January to December 2009 were not given self - efficacy training （ control group）, 45 （52 knees） given self -efficacy training （observation group）. General self- efficacy scale （GSES） was used to evaluate its self- efficacy, Hospital for Special Surgery knee score （HSS） to score knee joint, visual analogue scale （VAS） to assess knee joint pains. Before treat- ment, months 1, 3 after operation, patients were evaluated by GSES and HSS, and before treatment, 24 h, months 1, 3 after operation by VAS. Results There was significant difference in GSES between two groups （ F = 20. 2, P 〈 0. 05 ）, but not in time series （ F = 1.4, P 〉 0. 05 ） , not in interaction of time series （ F = 1.1, P 〉 0. 05 ）. There was significant difference in VAS between two groups （F= 11.2, P〈0. 05）, in time series （F=5.3, P〈0. 01） and interaction of time series （F= 1.7, P 〈 0. 05 ）. There was significant difference in HISS （ F = 13.5, P 〈 0. 05 ）, in time series （ F = 6.4, P 〈 0. 01 ）, but not in interaction of time series （ F = 1.3, P 〉 0. 05 ）. Conclusion Self - efficacy training is of some use for releasing postoperative pains and improving joint function in TKR patients.
