摘要
目的:评价系统性红斑狼疮(SLE)患者的抗双链DNA(dsDNA)抗体、抗核小体抗体(AnuA)、抗Sm抗体及抗C1q抗体的检测,分析这四种自身抗体对SLE的诊断价值。方法:选择SLE组患者72例、其他结缔组织病患者58例和健康体检人群50例,取其血清样本进行检测,采用ELISA法测定抗dsDNA抗体、AnuA、C1q;以免疫印迹法测抗Sm抗体,并对结果进行分析。结果:SLE患者的抗dsDNA抗体、AnuA、抗Sm抗体、C1q敏感性为45.2%、32.3%、29.0%、43.5%,特异性为94.6%、91.1%、96.4%、92.9%,四者联合检测敏感性达到64.5%,特异性达到89.3%,显著高于单个检测的敏感度(P<0.05)。结论:抗dsDNA抗体、AnuA、抗Sm抗体、C1q诊断SLE特异性高,但敏感性低,联合检测可以提高对SLE诊断的敏感性和诊断效率。
Objective: The evaluation of patients with systemic lupus erythematosus ( SLE), anti - double - stranded DNA ( dsD- NA) antibodies, antinuclear antibodies (AnuA), anti - Sm antibodies and anti - Clq antibody detection, analysis of these four au- toantibodiesthe diagnostic value of SLE. Methods : 72 cases of SLE patients, 58 cases of other connective tissue diseases and healthy people 50 cases, whichever serum samples to detect adopt the ELISA assay anti -dsDNA antibody AnuA, Cl q; immunoblotting measured anti - Sm antibodies and the results were analyzed. Results: SLE patients With anti - dsDNA antibody, AnuA, anti - Sm antibodies, Clq sensitivity of 45.2%, 32. 3%, 29.0%, 43.5%, specificity 94. 6%, 91.1%, 96.4%, 92. 9%, four joint detection sensi- tivity 64. 5%, specificity of 89. 3%, significantly higher than that of a single detection sensitivity ( P 〈 0. 05). Conclusion : The anti- dsDNA antibody AnuA anti -Sm antibodies, of Clq diagnosis SLE specificity, but low sensitivity, joint detection can improve the sensitivity and efficiency of diagnosis of SLE diagnosis.
