摘要
目的探讨乙肝病毒大蛋白(LHBs)的检测对于反映乙型肝炎病毒复制与活动的临床意义。方法采用酶联免疫吸附法(ELISA)检测260例慢性乙型肝炎患者血清中乙肝病毒大蛋白、乙型肝炎病毒e抗原(HBeAg)和乙型肝炎病毒前S1抗原(HBV-PreS1),实时荧光定量PCR法检测乙型肝炎病毒脱氧核糖核酸(HBV DNA),全自动速率法测定谷氨酸氨基转移酶(ALT)和天冬氨酸氨基转移酶(AST),并对血清中LHBs含量与HBV DNA水平变化进行相关性分析。结果 260例慢性乙型肝炎患者血清中LHBs与HBV DNA均有较高的阳性检出率(59.23%,67.69%),两者的阳性率差异无统计学意义(χ2=4.014,P>0.05),LHBs的阳性率均高于HBeAg和HBV PreS1阳性率(39.61%,32.69%),差异均有统计学意义(χ2值分别为20.010和36.864,均P<0.01)。不同HBV DNA拷贝数组之间,血清中LHBs含量差异有统计学意义(F=23.90,P<0.01)。LHBs水平与HBV DNA拷贝数对数值之间呈正相关(r=0.78,P<0.01)。LHBs阳性患者的ALT、AST水平明显高于LHBs阴性患者。结论 LHBs与HBV DNA有较高的符合率,LHBs可作为检测慢性乙型肝炎患者病毒复制与活动的进一步补充。
Objective To investigate the clinical significance of detection of serum hepatitis B virus large surface protein(LHBs) and HBV -DNA in patients with hepatitis B. Methods A total of 260 CHB patients were recruited in the study. Serum LHBs, Hepatitis B e antigen (HBeAg) and Pre -S1 antigen of hepatitis B virus( HBVPreS1 ) were measured by enzyme -liked immunosorbent assay (ELISA). Serum HBV -DNA level was quantitatively detected by real -time polymerase chain reaction(PCR). Serum ALT and AST were detected by automatic biochemical analyzer. Results All patients in the study showed high positive rate in LHBs and HBV DNA. Of the 260 patients, 59.23% were found LHBs and 67.69% were found HBV - DNA. Serum LHBs was significantly higher in patients as compare to those HBV -DNA(39.61%,χ2 = 20. 010) and HBVPreS1 (32.96% ,χ2 = 36. 864) in patients, the difference was statistically significant( P 〈 0.01 ). The difference of serum LHBs in different HBV - DNA level groups (F = 23.90 ) had statistically significant( P 〈 0.01 ). Serum LHBs showed direct correlation with HBV - DNA level(r =0.78, P 〈0.01 ). The LHBs posi- tive patients'ALT, AST markers were higher than those in the HBV - LP negative patients. Conclusion LHBs showed a high coincidence rate with HBV - DNA. Monitoring serum LHBs is a good adding test for diagnosing HBV replication and active.
