摘要
探讨胰岛素样生长因子 1受体 (IGF 1R)过度表达与乳腺癌术后、放射治疗后复发的相关性。方法 回顾性研究 2 5例乳腺癌术后、放射治疗后复发的病例 (复发组 ) ,同时设对照组5 0例。采用免疫组织化学的方法测定肿瘤组织的IGF 1R表达情况 ,分为 -、+、+ +、+ + + ,并将+ +和 + + +定义为过度表达。结果 复发组的过度表达率明显高于对照组 ,分别为 76 % ,2 2 % ,二者差异有极显著意义 (P <0 .0 1)。IGF 1R的表达与肿瘤大小、淋巴结转移情况、TNM分期无明显相关性。在全部病例中 ,IGF 1R过度表达者的 3、5年生存率显著降低 (P =0 .0 0 0 3) ;但在复发组中进一步分析生存情况 ,结果并未发现IGF 1R过度表达对生存有显著影响 (P =0 .5 5 10 )。结论 本研究从临床方面初步证实了IGF 1R与乳腺癌术后、放射治疗后复发的相关性 ;提示IGF 1R过度表达与乳腺癌细胞的放射抗拒有关 ,可能是一个预测放射敏感性的重要指标。
Objective To evaluate the clinical significance of insulin-like growth factor-1 receptor (IGF-1R) expression in the recurrence of breast cancer patients after postoperative irradiation. Methods The breast cancer data base was from patients treated by surgery and postoperative radiotherapy from 1986 to 1994. There were 25 breast cancer patients who had loco-regional recurrence within the irradiated fields (local recurrence group), while a control group of 50 breast cancer patients was chosen according to a match ratio of 2∶1. The two study groups had comparable distributions including matching with a ratio of 2∶1 with respect to age, menopausal status, primary tumor size, nodal status, TNM stage, surgery manner, radiotherapy technique and adjuvant chemotherapy. The IGF-1R expression on the parraffin-embedded slide was tested by immunohistochemistry. Each slide was rated on four scales: -,++,or ++,+++, of which the two latter were considered as overexpression. Results It was noted statistically that there was a significant higher rate of positive expression of IGF-1R in the local recurrence group than in the control group (P< 0.01),and the difference in overexpression of IGF-1R between these groups was extremely significant (76% vs.22%,P=0.0001).However, IGF-1R expression was not correlated to tumor size, nodal status and TNM stage ,so its influence on survival warrants further studies.Conclusions In this matched case control study, overexpression of the IGF-1R is closely associated with loco-regional recurrence in breast cancer treated by surgery and radiotherapy.
