摘要
目的探讨糖尿病、高血压等常见病对于肾结石发病年龄的影响,从而为临床进一步的预防及治疗提供依据。方法采用回顾性分析,收集2007年1月至2008年12月于第二军医大学第一附属医院泌尿外科住院治疗并首次诊断为肾结石患者的病例资料。分析糖尿病、血糖水平及高血压等常见代谢异常对于肾结石首次发病年龄的影响。结果共采集1291例患者资料。其中男性873例,女性418例。首次发病年龄较为集中于30~70岁,尤以50~60岁为多。其中单纯泌尿系结石患者930例(平均发病年龄62.2±9.6岁),合并高血压患者284例(52.7±9.2),合并糖尿病患者34例(58.5±9.5),合并糖尿病高血压患者43例(59.8±9.0)。将是否合并糖尿病及高血压作为分组依据时,是否罹患高血压对于发病年龄的影响具有统计学意义(P〈O.05)。而将空腹血糖分为正常、空腹血糖受损及糖尿病三组时,其首次发病年龄无统计学差异(P〉0.05)。组内可见血糖正常组发病年龄集中于60岁以后,而糖尿病组则集中于40~50岁。结论合并患有高血压可使得肾结石患者首次发病的年龄显著提前。糖尿病及血糖水平可能也有一定影响。在临床工作中,我们应该更加早期关注高血压、糖尿病患者的泌尿系统健康。
Objectives To explore the impact of diabetes and hypertension on the age of onset of urinary calculi, and thus provide a basis for further clinical prevention and treatment. Methods A retrospective analysis, we collected the clinical data of patients who was diagnosed urinary stones and hospitalized in the Second Military Medical University First Affiliated Hospital from January 2007 to December 2008. Analysis the effects of the common metabolic abnormalities of diabetes, blood sugar levels and high blood pressure on the patients'age of onset of kidney stone for the first time. Results Were collected the data of 1291 patients, 873 men and 418 women. The first age of onset is more concentrated in the 30 - 70 years old, especially in the 50 - 60 years of age. 930 patients with only urinary stone ( average age of 61.2± 9.6 years), 284 patients with hypertension (53.7 ± 13.2 ), 34 patients with diabetes mellitus (58.5± 9.5 ) , 43 patients with hypertension and diabetes mellitus (59.8 ± 9.0). When we took diabetes and hypertension as grouping criteria, we found that suffering from high blood pressure would make the onset of nephrolithiasis earlier, which is statistically significant ( P 〈 0.05 ). When we divided the Fasting blood glucose as normal, impaired fasting glucose and diabetes, the patients' age of onset of kidney stone for the first time was no significant difference( P 〉 0.05 ). But we noticed that patients in normal blood glucose group are concentrat- ed after the 60 years of age, while the diabetes group concentrated in the 40 - 50 years of age. Conclusions Suf- fering from high blood pressure would make the onset of nephrolithiasis earlier. Diabetes and abnormal blood sugar levels may also have some impact. In clinical work, we should pay attention to the urinary system health of the patients who suffering from hypertension and diabetes at an early stage.
