摘要
目的探讨脐血IgE、IL-4、IL-12水平对婴儿变态反应性疾病发生的预测价值及三者之间的相互变化关系。方法应用美国IMX型化学发光检测仪,采用免疫荧光方法测定129例新生儿脐血IgE水平;采用双抗体夹心ELISA法测定新生儿脐血IL-4、IL-12水平,并进行3 a随访。根据是否出现变态反应情况分为变态反应组和正常组。结果变态反应组脐血IgE、IL-4水平均较正常组明显增高(Pa<0.01),而IL-12水平较后者明显降低(P<0.01)。变态反应性疾病患儿脐血的3种介质相关分析显示IL-4与IgE呈显著正相关(r=0.878,P<0.01);IL-12与IgE呈显著负相关(r=-0.643,P<0.01);IL-12与IL-4呈显著负相关(r=-0.711,P<0.01)。结论脐血IgE、IL-2、IL-5水平检查可预测婴儿变态反应性疾病的发生,从而发现高危儿,对儿童避免变态反应性疾病的发生和指导医疗保健有重要意义。
Objective To explore the diagnostic value and relationship of cord blood immunoglobulin E ( IgE), interleukin - 4 ( IL - 4), interleukin - 12 (IL - 12 ) in infants with allergic diseases. Methods The levels of cord blood IgE in 129 newborns were measured with im- munofluorescence method by using IMX Chemiluminescence Apparatus. The levels of IL- 4 and IL- 12 were measured with double antibody sandwich ELISA method. All infants were followed up for 3 years. The patients were divided into the allergic diseases group and normal group according to the follow - up. Results The levels of IgE and IL - 4 in infants with allergic diseases were much higher than those in infants without allergic diseases ( P 〈 0.01 ). The levels of IL - 12 in infants with such diseases were much lower than those in infants without aller- gic diseases ( P 〈 0.01 ). Among levels of IgE, IL - 4 and IL - 12, there was a significant correlation between every 2 indexes. There was a significant positive correlation between IL- 4 and IgE( r = 0.878, P 〈 0.01 ). There was a significant negative correlation between IL- 12 and IgE ( r = - 0. 643, P 〈 0.01 ). There was a significant negative correlation between IL - 12 and IL - 4 ( r = - 0.711, P 〈 0.01 ). Conclusions The incidence of allergy diseases in infants can be predicted and high - risk infants can be found by determining the levels of cord blood IgE, IL-4 and IL- 12. It is of great significance to prevent allergy diseases in infants and improve health care for children.
