急性特发性血小板减少性紫癜患儿细胞免疫功能的变化 被引量:11

Study on Cellular Immunity Function of Children with Acute Idiopathic Thrombocytopenic Purpura

摘要 目的探讨急性特发性血小板减少性紫癜(ITP)患儿T细胞亚群、自然杀伤T细胞(NKT细胞)及Th相关细胞因子的变化。方法随机选取2009年3月-2010年5月在本院住院的30例急性ITP患儿为研究对象。男21例,女9例;年龄(2.50±2.39)岁。采用流式细胞仪检测其外周血T细胞亚群、NKT细胞、IL-2、IL-4、IL-6、IL-10及TNF-α、干扰素-γ(IFN-γ)水平,并以30例体检健康儿童为健康对照组进行比较。采用SPSS10.0软件进行分析。结果 ITP组外周血CD4+T细胞[(31.35±10.26)%]显著低于健康对照组[(37.29±8.18)%](P<0.05)。ITP患儿外周血NKT细胞[(0.59±0.42)%]显著低于健康对照组[(1.18±0.55)%](P<0.01)。ITP患儿外周血Th1相关细胞因子TNF-α、IFN-γ、IL-2表达分别为(5.23±2.18)ng·L-1、(5.58±2.49)ng·L-1、(29.78±8.08)ng·L-1,显著高于健康对照组[(1.98±0.82)ng·L-1、(2.31±0.49)ng·L-1、(21.64±11.99)ng·L-1](Pa<0.01);Th2相关细胞因子IL-4、IL-6、IL-10表达量分别为(2.34±1.15)ng·L-1、(4.26±1.55)ng·L-1、(2.56±1.01)ng·L-1,显著低于健康对照组[(6.03±1.11)ng·L-1、(6.49±1.39)ng·L-1、(4.61±1.30)ng·L-1](Pa<0.01)。结论急性ITP患儿存在明显的细胞免疫功能紊乱。 Objective To assess the changes of T cell subsets, natural killer (NK) T cell and Thl/Th2 related cytokines of children with acute idiopathic tbrombocytopenic purpura (ITP). Methods A total of 30 ITP patients[ (21 male,9 female;age (2.50 ±2.39) years] who were hospitalized from Mar. 2009 to May 2010, were randomly selected as the research objects, and 30 cases of healthy children were selected as the healthy control group. The levels of T cell subsets, NK T cell, Thl/Th2 related cytokines ( IL - 2, IL - 4, IL - 6, IL - 10) , tumor necrosis factor α( TNF - α) and interferon -γ ( IFN - γ) of all samples were stained with flow eytometry. Statistical analysis was made by using SPSS 10.0 software. Results The rate of CD4 + T cells [ ( 31.35 ± 10.26) % ] in peripheral blood in ITP group was significantly lower than that in healthy control group [ (37.29 ±8.18 )% ] (P 〈 0.05 ). The rate of NK T cells in peripheral blood in ITP group [ (0.59 ± 0.42 )% ] was significantly lower than that in the healthy control group [ ( 1.18 ± 0.55 ) % ] ( P 〈 0. 01 ). The levels of Thl related cytokines ( TNF -α ,TFN -γ and IL-2) in peripheral blood in ITP group [ (5.23 ±2.18) ng·L^-1,(5.58±2.49) ng ·L^-1 ,(29.78 -+8.08) ng·L^-1] were significantly higher, respectively, than those in healthy control group [ ( 1.98 ± 0.82) ng·L^-1, (2.31 ± 0.49) ng·L^-1, ( 21.64 ±11.99 ) ng·L^-1] ( P, 〈 0. 01 ). The levels of Th2 related cytokines ( IL - 4, IL - 6, IL - 10) in peripheral blood in ITP group [ ( 2.34± 1.15 ) ng·L^-1,(4.26±1.55) ng·L^-1,(2.56±1.01) ng·L^-1] were respectively lower than those in healthy control group [(6.03±1.11) ng·L^-1,(6.49 ±1.39) ng·L^-1 ,(4.61 ±1.30) ng·L^-1 ,p 〈0.01). Conclusion All of these may suggest the disorder of cellular immunity in children with acute ITP.
