摘要
目的探讨大剂量糖皮质激素联合弥可保治疗急性视神经炎的临床效果。方法对52例(80眼)确诊为急性视神经炎患者的临床资料进行回顾性研究,按治疗方法不同随机分为对照组和治疗组。治疗组28例(42眼)给予地塞米松20 mg静脉滴注,逐渐减量治疗,对照组24例(38眼)患者使用地塞米松10 mg静脉滴注,逐渐减量治疗,所有患者都配合给予肌肉注射弥可保。观察两组治疗前后视力、眼底、视野、视觉诱发电位的变化,随访3年,观察两组患者的临床疗效并进行对比研究。结果所有患者视力均较前提高,大剂量地塞米松联合弥可保治疗组治愈率为52.4%,总有效率为88.1%;对照组治愈率为36.8%,总有效率为78.9%。与对照组相比较治疗组显效率高(P<0.05),患者视功能恢复快。结论大剂量地塞米松联合弥可保治疗急性视神经炎疗效确切,可以缩短病程,不良反应少,是一种安全有效的治疗方法,值得临床推广。
Objective To investigate the curative effect of high-dose glucocorticoids combined with insurable on acute optic neuritis （AON）. Methods Fifty-two cases （ 80 eyes） of AON in our hosptial were randomly divided into control group and treat- ment group. Twenty-eight patients（42 eyes） in the treatment group were given dexamethasone 20 mg intravenous drip, and 24 cases（ 38 eyes） in the control group were given dexamethasone 10 mg intravenous drip, gradually reduction treatment, all of the patients were given insurable muscular injected. Eye fundus, view field and visual evoked potential before and after the treatment were observed after follow-up for 3 years, the clinical efficacy were observed and compared in the two groups. Results Visions of all patients were improved greatly, the cure rates was 52.4% in control group and 36. 8% in control group, the total effective rate was 88.1% in treatment group and 78.9% in the control group. Compared with the group ,the obvious effective rate was high- er and the average healing time was shorter in treatment group （P 〈 0. 05 ）. Conclusion The curative effect of high-dose glu- cocorticoids combined with insurable on acute optic neuritis （AON）is good. It can shorten the duration of symptoms, has fewer side effects. It is a safe and effective treatment methods and worthy of clinical promotion.
