摘要
目的探讨外源性补充谷氨酰胺对急性肠道移植物抗宿主病(GVHD)治疗的影响。方法将22例急性肠道GVHD患者随机分为标准化全胃肠外营养组(标准组,n=8)和加用谷氨酞胺的全胃肠外营养组(谷氨酰胺组,n=14),在出现肠道GVHD的第1天开始给予全胃肠外营养。结果两组患者摄入的营养物质基本相同,谷氨酞胺组好转时间为16d(6～31d),巨细胞病毒感染4例(29%),标准组好转时间为19天(10～25d),发生6例(75%)肠道巨细胞病毒感染。结论外源性补充谷氨酞胺能缩短急性肠道移植物抗宿主病好转时间,减少巨细胞病毒感染,有利于急性肠道GVHD患者恢复。
Objective To investigate the effects of extrinsic glutamine on patients with intestinal-type acute graft versus host disease（aGVHD）. Methods 22 patients with intestinal-type aGVHD were randomly divided into the group of standard total parenteral nutrition（standard TPN, n= 8） and the group of standard TPN with Gln（Gln group,n = 14）, Parenteral nutrition was initiated at the day after intestinal-type aGVHD （day 1）. Results Nutrient intake was similar in both groups. The recovery time of Gln group was at 16 days（6-31d）,four（29%）cases of CMV infection happened. The recovery time of standard TPN was at 19 days（10-25d） ,and six（75%）cases of CMV infection happened. Conclusion Extrinsic glutamine can shorten the recovery time on patients with intestinal-type aGVHD, diminishing the incidence of CMV.
