精氨酸对食管癌术后患者蛋白代谢、体液免疫的影响研究 被引量:1

Effect of Arginine Enhanced for Parenteral Nutrition on Nutrition Status and Humoral Immune in Patients After Esophageal Cancer Surgery

摘要 目的评价免疫营养素精氨酸(Arg)强化的肠外营养(PN)对食管癌患者手术后蛋白代谢和体液免疫功能的影响。方法筛选食管癌手术患者30例,随机分成2组:单纯静脉营养(Rt)组,精氨酸强化组(Arg组)。术后1~7d给予等氮量、等热量静脉营养,精氨酸强化组给与Arg 30g/d,分别于术前,术后d1、d8检测白蛋白(ALB)、前白蛋白(PAB)、转铁蛋白(TRF)以及体液免疫指标IgA、IgG、IgM。结果术后d8与d1相比PAB、TRF、IgA、IgG以及IgM有显著性升高升高(P<0.05),实验组指标与Rt相比有显著性差异(P<0.05)。结论食管癌手术患者后应用Arg增强的PN与常规PN相比能较好地恢复体液免疫功能,改善蛋白代谢。 Objective To determine the effect of Arginine enriched parenteral nutrition on nutrition status and Humoral immune function following esophageal cancer surgery.Methods 30 patients who underwent the same elective operation for thoracic esophgeal carcinoma were prospectively and equally randomized into 2 groups: Arg group and standard PN group.PN was infused to all patients with equal calorie and equal nitrogen by central vein during the immediate 7d after operation.30g/d Arginine was added into PN in the Arg group,On preoperation and on postoperative day 1,8,ALB,PAB,TRF,IgA,IgM and IgG were determined Results On the eighth day of postoperation,all indicators of Arg group significantly better than routine group except for ALB(P〈0.05),Conclusion Compare with routine PN,the nutrition status and humoral immune response of patients after esophageal cancer surgery can be ameliorated by Arg enhancing PN.
