摘要
目的:探讨应用三维适形放射治疗局部复发鼻咽癌的治疗效果。方法:2006年5月至2009年9月对26例局部复发鼻咽癌患者行三维适形放射治疗。全组病例均采用5-7个共面或非共面固定适形野等中心照射,时间-剂量-分次处方:65-70Gy/26-28次,共5-5.5周。结果:再程放疗后随访期内死亡1例,局部完全消退17例(65.4%),部分缩小6例(23.1%),稳定2例(7.8%),局部控制率88.5%。再程放疗所致急性放射反应较少而轻。结论:应用三维适形放射治疗技术治疗局部复发鼻咽癌具有较明显的剂量分布优势,放射反应轻且并发症少,肿瘤局部控制率高,远期疗效和后遗症还有待观察。
Objective：To evaluate the treatment results of locally recurrent nasopharyngeal carcinoma（NPC） with there-dimensional conformal radiation therapy（3-D CRT）.Methods：From May 2006 to September 2009,a total of 26 cases of locally recurrent nasopharyngeal carcinoma were treated by 3-D CRT.Among 5-7 static conformal portals were delivered with coplanar or noncoplanar isocentric irradiation technique.The prescription dose-fractionation-time was 65-70Gy/26-28F/5-5.5W.Results：In the follow-up time,1 patient died.Partial tumor regression 17 cases（65.4%）,the tumor reduced in 6 cases（23.1%）,tumor stabilization in 2 cases（7.8%）.No evidence of local failure was found in 88.5%.The acute and self-limiting complications from reirradiation were slight and few.Conclusion：3-D CRT,for locally recurrent NPC,had an obvious advantage on dose distribution,and a high local control rate low side reaction rate.But its long term curative effect need to be further observed.
