摘要
目的探讨手部皮神经痛的诊断与治疗方法。方法回顾性分析我院门诊确诊为手部皮神经痛40例患者资料,诊断依据是疼痛的部位及查体所见的痛觉改变区域与各神经在手部的分布区一致;治疗采用强的松龙混悬液1 mL(25 mg)加2%利多卡因5 mL注射至各神经分支穿过深筋膜处,1周后不愈者再行第2次注射,少数于3周后再注射第3次。结果所有病例皆在注射后10 min内止痛,检查神经分布区痛觉丧失,数小时后多数病例再现轻痛。1周后26例痊愈,10例经第2次注射后痊愈,4例经第3次注射后痊愈。结论疑为手部神经皮支神经痛的病例经排除颈、肩、上臂及前臂可能引致神经痛的病变后,采用强的松龙注射神经穿过筋膜方法治疗,简便易行,疗效可靠。
Objective To introduce diagnosis and treatment for neuralgia of cutaneous nerves of the hand. Methods The clinical data of 40 cases with cutaneous nerve neuralgia in outpatient department of our hospital were collected and analyzed. The diagnosis was made ac- cording to the scope of pain change,which was identical to the territory of certain cutaneous nerve branch. Local injection of 1 mL （25 rag） suspension of prednisolone plus 5 mL of 2% lidocaine to where the nerve passing through the deep fascia was given. The second injection was given if the patient did not recover one week later. The third injection was given if necessary two weeks later. Results The pain was stopped within 10 minutes after injection in all cases. The skin pain sensation lost in the area of the nerve distribution. In most cases the pain reap- peared a few hours later,but in mild degree. One week later,26 cases recovered, 10 cases recovered after the second injection,4 cases recov- ered after the third injection. Conclusion Local injection of prednisolone to where the nerve pass through the deep fascia in treating suspec- ted cases of neuralgia of cutaneous nerves of the hand is feasible and effective after ruling out the neuralgia induced by nerves at neck, shoul- der, arm or forearm
