摘要
目的 :探讨过氟化碳液体在复杂性视网膜脱离手术中的应用及预后。方法 :回顾 1997年 1月～ 1999年 9月间我院住院治疗的 18例 ( 18眼 )复杂性视网膜脱离的患者 ,应用玻璃体手术 ,术中采用过氟化碳液体 (过氟三丁烷胺 ,C12 F2 7N) ,术后行气 /液交换、硅油或膨胀气体充填。结果 :18例患者经 3～ 6个月随访 ,视网膜最终复位者15眼 ( 83 3 % ) ;4眼采用C2 F6气体充填 ,气体吸收后 ,有 2例视网膜复位不良 ;14例采用硅油充填 ,1例因周边部视网膜粘连较重 ,术后 2周视网膜又脱离。结论 :术中应用过氟化碳液体 ,有机械性压迫作用 ,打开漏斗状视网膜脱离 ,展平视网膜及大裂孔翻转瓣 ,进行增殖膜清除 ,驱逐视网膜下液 ,使视网膜复位。过氟化碳液体在复杂性视网膜脱离手术中的应用为一种安全有效的工具。
Objective: To inquire into the application of perfiluorocarbon liquids in operation of complicated retinal detachment and prognosis.Methods: To review 18 cases (18 eyes) of complicated retinal detachment treated in our hospital from January 1997 to September 1999.Using perfiluorocarbon liquids (C 12 F 27 N)during operations of vitroretinopathy and gas/liquid exchange,silicone oil or inflate gas tamponade postoperation.Results: After 3 6 months of follow up,final retinal reattachment was 15 eyes (83.3%) in 18 eyes. 4 cases used C F 6 gas tamponade.After gas was absorbed,retinal re detachment happened in 2 cases.14 cases used silicone oil tamponaed.Because serious retinal coheres in periphery place,1 case happened retinal re detachment in the 2 week after operation.Conclusion: Using perfiluorocarbon liquids during operations,it has mechanical constriction effect.Open up the funnel retinal detachment.Unfold retina and reversal petal of giant tear.Help peeling retinal hyperplasia membrane.Displace subretinal fluid.Attach retina.it's safe and effective tool that applying perfiluorocarbon liquids in operation of complicated retinal detachment.
