拔牙与非拔牙矫治尖牙间宽度变化的Meta分析 被引量:2

Inter-canine width after extraction and non-extraction treatment: a meta-analysis

摘要 目的系统比较拔牙与非拔牙矫治对尖牙间宽度的影响。方法计算机检索PubMed、Embase、Cochranelibrary、中文科技期刊数据库(VIP)、中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、万方数据库,收集通过模型测量拔牙与非拔牙矫治对尖牙间宽度影响的所有文献,检索时间为1990年1月1日至2012年4月10日。由2名评价员独立地对纳入文献进行资料提取、质量评价并交叉核对,而后采用RevMan 5.1.4软件进行Meta分析。结果共纳入文献12篇,包括846例患者。Meta分析结果:①拔牙组与非拔牙组相比,前后对照试验(CBAs)Meta分析结果显示上颌尖牙间宽度的变化差异显著[WMD=0.61 mm,95%CI(0.41,0.81)];随机对照试验(RCTs)Meta分析结果显示组间无显著差异[WMD=-0.32 mm,95%CI(-1.41,0.77)];②下颌尖牙间宽度的变化前后对照试验(CBAs)及随机对照试验(RCTs)Meta分析结果均显示差异显著[CBAs:WMD=0.61 mm,95%CI(0.41,0.81);RCTs:WMD=0.81 mm,95%CI(0.23,1.39)]。结论拔牙矫治不会导致上颌尖牙间的宽度缩小,且拔牙矫治下颌尖牙间的宽度增大更多。 Objective To evaluate the changes of inter-canine width in consecutively treated extrac- tion and non-extraction patients. Methods All the randomized controlled trials (RCTs), controlled clinical trial (CCTs) and controlled before-and-after study (CBAs) about inter-canine width in consecutively treated extraction and non-extraction patients were collected by searching PubMed database, Embase, Cochrane librar- y, VIP database, CBM database, CNKI database and Wanfang database. All the widths were measured by models, and the retrieval time was from 1990-01-01 to 2012-04-10. The assessment of methodological quality and data extraction of the included studies were performed independently by two reviewers, and meta-analysis was conducted with RevMan 5.1.4 software. Results A total of 12 studies involving 846 patients met the inclusion criteria. Significantly differences were found in maxillary inter-canine width (WMD =0.61 mm, 95% CI =0.41 to 0.81 ) in the meta-analysis of CBAs, but not found in the analysis of RCTs ( WMD = -0.32 ram, 95% CI = - 1.41 to 0. 77 ). The mandibular inter-canine width was increased significantly for the extraction group compared with non-extraction group in the analysis of CBAs and RCTs( CBAs: WMD = 0. 61 mm, 95% CI =0.41 to0.81; RCTs: WMD=0.81 mm, 95%CI=0.23 to 1.39). Conclusion Extraction treatment will not lead to narrowing of the maxillary inter-canine width, and the mandibular inter-canine width is increased more for the extraction group.
