摘要
目的 为深入了解新生儿早期肾功、体液状态并寻找其检测方法以保护发育不完善的肾功能和维持体液平衡 ,对 12 2例新生儿早期 (0— 5天 )的血、尿溶质各成份尿渗透压、尿钠等进行测定并计算肾功指数 :内生肌酐清除率、钠排泄分数、渗透性溶质清除率、纯水清除率 ,结果表明早产、早产窒息儿尿溶质排泄明显高于足月儿 (对照组 ) ,窒息儿尿渗透压明显高于非窒息儿 ,且有显著差异 (P <0 .0 1)。早产窒息组溶质排泄分数、渗透性溶质清除率及纯水清除率均高于对照组。窒息组肾溶质负荷达正常范围值高限 ,故认为上述检测指标与早产儿肾发育成熟程度。
To investigatd the renal function and body fluid state of the early neonate and to find method of its detecting in order to protect the unwell develoep renal function and to maitain the body fluid balance.Solute components of blood and urine of 122 early neonate (0~5days) were measured.Um、UNa、RFI,including Ccr、FENa、Cosm、CH 2o were measured.Results showed that excretion of urine solute of preterm neonate and preterm asphyxiaed neonate was significantly higher than that of term neonate,Um of asphyxiated neonate was significantly higher than that of non asphyxiated neonate with significant difference between them(p<0 01).Solutes FE,Cosm and CH 2O of asphyxiated neonate were significantly higher than term neonate.renal soluter load of asphyxiated neonate was at the top of normal rage.Thesegor it is considered that the above mentioned measured indices are related to renal developmend and maturity of the preterm neonate and renal function dmnage of the asphyxiafed neonate.(Original article in page 106)
