摘要
目的探讨心血管临床药师如何在临床对患者开展药学监护。方法对1例慢性心力衰竭患者的药物治疗过程进行全程监护,总结经验建立监护模式。结果临床药师通过对患者状况评估,建立个体化用药监护计划及处理措施,并跟踪治疗,解决了药物治疗过程中存在的问题,同时也避免了药物治疗中潜在的风险。结论建立完善的药学监护模式,提高了患者治疗依从性和临床治疗效果,减少了药物不良反应。
Objective To investigate the pharmaceutical care model the cardiovascular clinical pharmacists adopted.Methods The whole medication process for one patient with chronic heart failure was monitored and the experience was analyzed to establish a care model.Results The clinical pharmacist established a personalized pharmaceutical care plan and treatment measures based on the patient＇s condition assessment and also followed up the patient for necessary treatment.This approach solved the problems in the drug treatment process,while at the same time avoided the potential risks in the drug treatment.Conclusion The establishment of a complete pharmaceutical care model improves both patient compliance with treatment and clinical outcomes and also reduces adverse drug reactions.
