摘要
目的:通过临床观察解郁静心颗粒联合氟西汀对围绝经期抑郁症患者HAMD抑郁量表评分、单胺类神经递质及内分泌功能的影响,探讨其疗效机理。方法:将119例围绝经期抑郁症患者随机分为观察组和对照组,观察组68例:采用中药解郁静心颗粒联合氟西汀胶囊治疗。对照组51例:单用氟西汀胶囊,治疗8周为1个疗程。于治疗前后分别对两组进行HAMD抑郁量表评分,检测血浆5-羟色胺(5-HT)、去甲肾上腺素(NE),血清E2、FSH、LH及B超测定子宫内膜厚度。结果:观察组总有效率95.58%(65/68),对照组总有效率为88.23%(45/51)。两组治疗4周后HAMD量表总分比较差异有显著性(P<0.05)。治疗8周后观察组血浆5-HT下降(P<0.01),NE升高(P<0.05),5-HT/NE比值下降(P<0.05)。结论:中药解郁静心颗粒联合氟西汀胶囊治疗围绝经期抑郁症的效果,优于单纯应用氟西汀胶囊;对调整围绝经期抑郁症患者体内神经递质的代谢水平强于单纯应用氟西汀胶囊;中西医联合能预防和减少药物不良反应的发生。其机制可能与以下作用有关:(1)调节单胺类神经递质的合成与释放;(2)协调神经内分泌功能;(3)调节生殖内分泌功能。本研究从诊断到疗效均进行一定量化观察其临床疗效,为围绝经期抑郁症的药物治疗提供理论依据。
Objective ： The mechanism of Jieyu Jingxin Granules combined with fluoxetine treating depression during pe fi menopausal period was discussed by observing the influence on HAMD depression scale, monoamine neurotransmitter and endocrine function. Methods：ll9 cases with depression during peri menopausal period were randomly divided into observation group and control group. 68 cases in observation group were treated with Jieyu Jingxin Granules combined with fluoxetine while 51 cases in control group with fluoxetine only. 8 weeks was a course. HAMD depression scale scores before and after treatment in two groups were compared. 5 HT, NE, E2, FSH and LH levels were measured and endome trial thickness was tested by type B ultrasonic method. Results：The total effective rate in observation group was 95.58% （65/68）, in control group was 88.23 % （45/51 ）. 4 weeks later, HAMD scores in two groups had obvious significance （P 〈 0.05 ）. 8 weeks later,5 HT level was decreased（ P 〈 0.01 ）, NE level was increased（ P 〈 0.05 ） ,the ratio of 5 HT/ NE was decreased（P 〈0.05）. Concl^ion：Jieyu Jingxin Granules combined with fluoxetine has better effect than single fluoxetine, has strong regulation effect on neurotransmitter of depression patients during peri menopausal period and has few side effects. The mechanisms may be related to regulating synthesis and release of monoamine neurotransmitter, coor dinating nem＇oendocrine function and regulating reproductive endocrine function. This study provided basis reference for medication of depression during peri menopausal period by quantitatively observing the diagnosis and effect.
