摘要
目的分析研究老年股骨转子间骨折在闭合复位下行股骨近端髓内钉螺旋刀片(proximalfemoral nail antirotation,PFNA)内固定治疗的效果。方法 76例老年股骨转子间不稳定型骨折患者在X线监视下,采用闭合复位、PFNA内固定治疗,并评估其疗效。结果本组76例,74例骨折完全愈合,2例骨不愈合并股骨头坏死;骨愈合但出现退钉2例,骨折端短缩1例,螺旋刀片切割1例。结论 PFNA具有微创、操作简单、内固定可靠、并发症少、骨折愈合率高的优势,是治疗老年股骨转子间骨折的理想方案。
Abstract： Objective To analyze the effect of closed reduction followed by internal fixation with proxi- mal femoral nail antirotation （PFNA） in elderly intertrochanteric fracture patients. Methods In 76 old pa- tients with unstable intertrochanteric fractures, the closed reduction and internal fixation of PFNA was underg- one under X-ray monitoring. Results In 76 cases of this group, 74 fractures healed and the other two suf- fered from the bone disunion and femoral head necrosis. And in the 74 bone union cases, there were 4 compli- cations, such as retreat nails in two cases, fracture shortening in one case, and spiral blade cutting in one case. Conclusion It is recommended of the PFNA for minimally invasion, simple operation, reliable fixation, fewer complications, and high fracture healing rate as an ideal solution for the treatment of senile intertro- chanteric fractures.
