摘要
目的观察自拟健脾降浊汤与西药联用治疗2型糖尿病胰岛素抵抗的临床疗效。方法将80例中医辨证为脾虚痰浊型的2型糖尿病胰岛素抵抗患者随机分为两组,对照组40例用二甲双胍肠溶片和瑞格列奈片治疗,治疗组40例在对照组基础上加服自拟健脾降浊汤,疗程为8周。主要观察治疗前后空腹血糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、胰岛素敏感性指数(ISI)、症状体征等指标的变化。结果治疗组和对照组临床有效率分别为85.0%、62.5%,两组疗效差异有统计学意义(P<0.05);在改善HbAlc、FINS、ISI和血脂方面,治疗组优于对照组(P<0.05)。结论健脾降浊汤联合西药治疗2型糖尿病胰岛素抵抗有良好的协同效应,能够降低血糖,改善胰岛素抵抗。
Objective To observe the clinical efficacy of integrative treatment with self-intended Jianpi Jiangzhuo Decoction and Western medicine on the insulin resistance in patients with type 2 diabetes. Methods Eighty patients,TCM Syndrome Differenti- ation Spleen phlegm, of type 2 diabetes patients with insulin resistance were randomly divided into two groups. The control group (40 cases) was treated with metformin enteric-coated tablets and repaglinide tablets;while the intended Jianpi Jiangzhuo Decoc- tion based on the treatment of the control group. The course of therapy was eight weeks. The changes of fasting blood glucose ( FBG), glycosylated hemoglobin Alc ( HbA1 c ), fasting insulin ( FINS ) , cholesterol ( TC ) , triglyceride ( TG), high-density lipopro- tein cholesterol(HDL-C) ,low density lipoprotein cholesterol (LDL-C), insulin sensitivity index (ISI), signs and symptoms and other indicators were observed before and after treatment. Results The clinical efficiency was 85% in the treatment groups and 62.5% in the control group,the difference between the two groups was statistically significant( P 〈 0.05 ). HbA1 c, FINS, ISI and lipids were improved obviously in the treatment group as compared with the control group ( P 〈 0.05 ). Conclusion Jianpi Jiang- zhuo Decoction combined with Western medicine for the insulin resistance in the patients with type 2 diabetes have a good syner- gistic effect in reducing blood sugar improving insulin resistance.
