摘要
目的:了解北京石景山医院门诊处方综合运用情况,完善我院门诊处方质量水平,减少因处方质量问题造成的药品不良反应、药源性疾病、医患纠纷等问题。方法:以随机抽样法抽取我院2010年1月—2011年12月门诊处方共14 948张逐一核查,对不合理处方进行汇总点评,点评内容包括处方前记和后记部分是否完整、药品使用合理性、药品适应证是否与诊断相符、药物选用是否合理、药物之间是否存在配伍禁忌、抗菌药物选择及用法与用量是否合理等。结果:不合理处方317张,占抽查总处方的2.12%。结论:医院应加强管理,医生应加强责任心及提高用药水平,药师应持续进行处方点评和处方干预,提高处方质量。
OBJECTIVE:To probe into the overall quality of the outpatient prescriptions in Beijing Shijingshan Hospital in order to improve the quality of outpatient prescriptions and reduce the incidences of adverse drug reaction, drug-induced disease and physician-patient disputes caused by low quality of prescriptions. METHODS:Between Jan. 2010 and Dec. 2011, a total of 14 948 outpatient prescriptions in Beijing Shijingshan Hospital were randomly sampled. The irrational outpatient prescriptions in which were subjected to comment with regard to the completeness of the foreword and postscript, the rationality in drug use and choice of drugs, the conformity of the indications with the diagnosis, the incompatibility between (among) drugs and the rationality in the selection, dosage & administration of antibiotics etc. RESULTS:317 (2. 12% ) of the outpatient prescriptions were found to be irrational. CONCLUSION: To improve the quality of prescriptions, it is important to tighten hospital management, strengthen clinicians' responsibility and improve the level of medication, and intensify pharmacists' comment and intervention on prescriptions.
