摘要
目的探讨不同备皮方式对于人工膝关节置换术(total knee arthroplasty,TKA)切口感染的影响,选择最佳备皮方式,以期降低手术切口感染率。方法选择2007-2011年因类风湿关节炎在我院行膝关节置换术的患者349例,所有备皮均在术前2h内进行,随机分为3组,第1组为葡泰组,120例患者使用2%葡泰(葡萄糖酸洗必泰)清洁手术野;第2组为温水组,157例患者使用温水清洁手术野;第3组为对照组,72例患者常规使用剃毛刀进行术前备皮。结果共发生切口感染29例,感染率为8.31%。其中葡泰组发生2例,感染率为1.67%;温水组13例,感染率为8.28%;对照组14例,感染率为19.44%。葡泰组与温水组比较χ2=5.81,0.01<P<0.05;葡泰组与对照组比较χ2=18.62,P<0.01;温水组与对照组比较χ2=5.92,0.01<P<0.05。结论葡泰较其他两种方式更有利于降低手术切口感染率。
Objective To discuss the influence of different methods of skin preparation to total knee arthroplasty (TKA) infection, so as to choose the best method of skin preparation to reduce the infection rate of operative incision. Method Select 349 patients who accepted TKA in our hospital for rheumatoid arthritis from 2007 to 2011, and divide them into three groups. All the skin preparations were completed two hours before the opera- ! tion. The first group was chlorhexidine gluconate group, and the operative sites of 120 patients were cleaned by 2% chlorhexidine glueonate solution; the second group wae warm water group, and the operative sites of 157 patients were cleaned by warm water; and the third group was control group, and the 72 patients used common razor to do skin preparation. Results 29 cases appeared incision infection, with the infection rate of 8.31%. Among which, 2 cases were in chlorhexidine gluconate group, with the infection rate of 1.67% ; 13 cases were in warm water group, with the infection rate of 8.28% ; and 14 cases were in control group, with the infection rate of 19.44%. Compared with chlorhexidine gluconate group and warm water group, X2 = 5.81,0. 01 〈 P 〈 0. 05; compared with chlorhexidine gluconate group and control group, X2 = 18.62, P 〈 0. 01; and compared with warm water group and control group, X2 =5. 92, 0. 01 〈 P 〈0. 05. Conclusion Chlorhexidine gluconate could reduce the infection rate of operative incision compared with other methods.
