邻指皮瓣和指动脉岛状皮瓣修复指端缺损的临床疗效对比分析 被引量:10

Comparative study on therapeutic effects between cross finger flap and reversed digital artery island flap for repairing distal defect

摘要 目的探讨邻指皮瓣与指动脉岛状皮瓣在修复指端缺损中的临床效果及优缺点。方法2009年5月-2010年5月50例指端缺损患者.缺损范围:0.8cm×1.0cm-1.5cm×2.5cm。根据病情应用邻指皮瓣修复25例,指动脉岛状皮瓣修复25例。术后随访1~1.5年,对患指整体外观、伸屈功能、皮瓣感觉恢复、两点辨别觉、有无出现痛觉过敏及患指耐寒性进行比较。结果46例皮瓣全部成活未出现坏死及水疱,4例指动脉岛状皮瓣出现瘀紫水疱,予拆除蒂部部分缝线及换药治疗。皮瓣成活:邻指皮瓣术后患指整体外观及耐寒性较指动脉岛状皮瓣好。对伸屈功能采用芹检验,结果表明两组差异有统计学意义(X^2=4.735,V=1,P=0.03)。对两点辨别觉采用两组独立样本的t检验得t=2.518.P=0.015,差异有统计学意义。对感觉恢复采用有序分类变量两组独立样本的秩和检验,得Z=2.456,P〈0,05。、按照α=0.05检验水准,拒绝H10。两组中位数间的差异具有统计学意义。结论在条件允许情况下应用邻指皮瓣修复指端缺损较指动脉岛状皮瓣具有更多优点. Objective To investigate the effect between cross finger skin flap and reversed digital artery island flap in the repair of distal finger defect. Methods Since May 2009 to May 2010, 50 cases of the distal finger defect were treated with different skin flaps, 25 patients with cross finger skin flaps and 25 patients with reversed digital artery island flap. The defect scopes were 0.8cm-l.0cm to 1.5cm-2.5cm. We compared digital appearance, flexion and extension functions, sense, two-point discrimination, hyperalgesia and cold resistance of two different skin flaps, through follow-up 1 year to 1.5 year after the operation. Results The cross finger skin flaps of 4 cases appear blister. The others survived. In the digital appearance and the digital cold resistance, the cross finger skin flap was better than the reversed digital artery island flap. For flexion and extension functions, a chi-square test showed that the difference was statistically significant (X^2 =4.735, v =1, P =0.03). For two-point discrimination, the t test of two independent-samples resulted in r =2.518, P=0.015, the difference was statistically significant (P〈0.05). Conclusion At conditions permitting, it has more advantage by using cross finger skin flap than that of reversed digital artery island flap.
