体外受精-胚胎移植中发生中重度卵巢过度刺激综合征危险因素分析 被引量:6

摘要 目的通过比较体外受精-胚胎移植(IVF-ET)治疗中发生中重度卵巢过度刺激综合征(OHSS)患者与非OHSS患者一般情况、治疗周期、用药情况等相关指标,筛选出其中的危险因素,以指导临床预防治疗工作。方法回顾分析接受(IVF-ET)治疗的不孕患者共635个周期,发生中重度OHSS 22例为OHSS组,选择同期接受IVF-ET治疗患者中未发生OHSS的患者30例为非OHSS组,比较2组的临床特征。结果 OHSS组患者启动日促卵泡生成素(FSH)、促黄体生成素(LH)水平高,人绒毛膜促性腺激素(HCG)日雌二醇(E2)水平高、HCG诱排剂量大,与非OHSS组比较差异有统计学意义(P<0.05)。结论启动日FSH、LH水平,HCG日E2水平,对中重度OHSS发生有预测作用。 Objective To compare the correlative parameters about general run of things, treatment cycle situation, cure situation of the moderate and severe OHSS patients and non - OHSS patients, whose pregnancy was assisted by the technology of in vitro ferti- lization and embryo transfer, to screen the risk factors and direct the clinical prevention and cure work. Methods 635 cycles of IVF - ET were analyzed retrospectively. 22 cycles took place in the moderate and severe OHSS patients as OHSS group, 120 patients in non - OHSS as control group by random selecting from the corresponding period treatment. The non - OHSS patients were classified into less than 10, 10 -20 and more than 20 three groups according to the number of ooeytes. The clinical characteristics were compared among each group of them. Results OHSS patients had more basal antrum ovarian follicles and more 10mm ovarian follicles, higher LH after down - treatment, higher E2 after HCG treatment, while less Gn which were significantly different between the OHSS group and the non - OHSS group ( P 〈 0.05 ). Conclusion The basal antrum ovarian follicles, LH after treatment, E2 level after HCG treatment and more than 10mm ovarian follicles are predictors to OHSS. Preventive treatment to high risk patients can lighten the occurrence of OHSS.
