摘要
目的比较分析促性腺激素释放激素拮抗剂(GnRH-Ant)方案与GnRH-a长方案在IVF-ET患者中的临床应用。方法选取使用GnRH-Ant方案38例患者作为研究组,黄体期长方案76例患者作为对照组,对2组的年龄、不孕年限、获卵数、促性腺激素(Gn)天数及实验室结果及妊娠结局进行分析。结果研究组与对照组年龄(34.74±4.42)与(31.88±3.82)岁,Gn用量(21.89±12.12)与(29.51±13.60)支,获卵数(4.61±3.98)与(10.14±6.31)个差异均有统计学意义(P<0.01);2组不孕年限(5.61±3.94)与(4.87±3.52)年,Gn天数(9.18±2.59)与(10.08±2.12)d,移植胚胎数(2.08±0.78)与(2.09±0.80)个的比较,差异均无统计学意义(P>0.05)。研究组与对照组的优胚率(65.6%)与(53.2%)(P<0.01),妊娠率(29.9%)与(48.48%),差异有统计学意义(P<0.05);受精率(77.7%)与(72.7%),卵裂率(94.1%)与(94.9%),流产率(12.5%)与(27.3%),差异均无统计学意义(P>0.05);卵巢过度刺激综合征(OHSS)率分别为2.56%、5.56%。结论 GnRH拮抗剂方案与GnRH-a长方案相比可能具有更好的临床应用优势。
Objective To compare the clinical application between antagonist （ GnRH - Ant） protocol and GnRH - a long pro-tocol in IVF - ET and to seek more reasonable, economical and effective treatment program. Methods 38 cases applied GnRH - ant protocol as a research group and 76 patients that applied long protocol as control group. Pregnancy outcome of two groups were analyzed according to age, years of infertility, the number of oocytes retrieved, Gn days and embryo development. Results It was found that there were significant differences in the two groups in age （34.74±4.42 vs. 31.88± 3.82） , Gn dosage （21.89 ± 12.12 vs. 29.51 ±13.60） , number of oocytes retrieved （4.61 ± 3.98 vs. 10.14± 6.31 ） （ P 〈 0.01 ） ; There was no significant difference between two groups in years of infertility （5.61 ±3.94 vs. 4.87± 3.52）, Gn days （9.18±2.59 vs. 10.08±2.12）, numbers of embryo trans- ferred （2.08±0.78 vs. 2.09±0.80） （P 〉0. 05 ）. Two groups had a statistically significant difference in high -quality embryo rate （65.6% vs. 53.2%） （P〈0.01） and pregnancy rate （29.9% vs. 48.48%） （P〈0.05）, but fertilization rate （77.7% vs. 72.7% ）, cleavage rate （94. 1% vs. 94.9% ）, abortion rate （ 12.5% vs. 27.3% ） showed no significant difference （P 〉0.05）. OHSS incidence of the two groups was respectively 2.56% and 5.56%. Conclusion Comparing with GnRH - a long protocol, the Gn- RH antagonist protocol may have a better clinical application advantages.
