摘要
目的探讨糖皮质激素受体(GR)基因亚型表达在婴儿痉挛症(IS)患儿发病机制中的作用。方法采用反转录聚合酶链反应(RT-PCR)检测IS患儿及正常对照儿童外周血单核细胞(PBMC)sGRα和βmRNA的表达,并分析其与IS患儿对糖皮质激素(GC)的疗效相关性。结果 30例患儿中,对GC治疗显效者19例,有效率为63.3%。无效者11例,占36.7%。IS组PBMCs GRαmRNA的表达水平低于正常对照组(P<0.05);显效组GRαmRNA的表达水平与无效组比较差异无显著性(P>0.05)。IS组GRβmRNA的表达水平与正常对照组无显著性差异(P>0.05);显效组GRβmRNA的表达水平与无效组无显著性差异(P>0.05)。结论 GRαmRNA的降低可能参与了IS的发病机制,未发现GC治疗IS疗效与GR mRNA表达水平之间的关系。
Objective To investigate the expression of glucocorticoid receptor-alpha and beta(GRα and GRβ) mRNA in peripheral blood mononuclear cells (PBMCs) in patients with infantile spasms (IS) and to analyze their association between the pathogenesis and clinical response to glucocoticoid therapy. Methods The expression of GRα and GRβ mRNA in PBMCs from 30 nomal controls and 30 patents with IS was detected by reverse transcription polymerase chain reaction (RT-PCR) respectively. We analysed the relationship between the therapeutic effects and expression level of GRα and GRβ mRNA.Results Among 30 cases of IS, 19 cases (63.3%) had achieved remission or I〉50 % seizure reduction with glucocoticoid therapy. Compared with normal controls, the expression of GRαmRNA in the IS was decreased (P〈0.05), but there were no significant differences between GC sensitive group and GC resistant group (P〉0.05), between the IS and the normal controls and between GC sensitive group and GC resistant group in the expression of GRβ mRNA (P〉0.05). Conclusion The expression of GRαmRNA might play an important role in the pathogenesis of IS. We did not found the association between the therapeutic effects and expression level of GR mRNA.
