摘要
目的用Meta分析方法评价阿托伐他汀治疗不稳定心绞痛的临床疗效和对不稳定心绞痛患者血脂的影响。方法以"阿托伐他汀"、"不稳定心绞痛"为关键词对CNKI数据库、万方数据库2001～2011年的文献进行检索,利用RevMan 5.0软件进行合并效应和异质性分析;文献同质时采用固定效应模型,否则采用随机效应模型。临床疗效的效应尺度用机会比OR(Odds Risk)和95%的可信区间(95%CI)表示,血脂变化的效应尺度用加权均数差(WMD)和95%的可信区间(95%CI)表示;用漏斗图评估发表偏倚;临床疗效采用有效和无效两种结果。结果临床疗效的OR合并值为6.45,95%的可信区间为4.54～9.15;TC、LDL-C、HDL-C、TG的合并WMD分别为-1.03、-0.89、0.13、-0.4,95%的可信区间分别为-1.31～-0.75、-1.15～-0.63、0.02～0.24、-0.58～-0.21。结论研究显示阿托伐他汀治疗不稳定心绞痛临床疗效明显;可有效降低患者血液中TC、LDL-C、TG的浓度,升高HDL-C的浓度,改善患者血脂异常状,显著降低心血管事件的发生。
Objective To evaluate the effect of atorvastatin on unstable angina pectoris（UAP） and its influence on patients＇ blood fat by Meta - analysis. Methods Literatures during 2001 - 2011were searched from CNKI and Wanfang database using atorvastatin and UAP as the key words. The total effect and the heterogeneity were analyzed by RevMan 5.0 software, homogenous literatures with fixed - effect model and heterogenous with random - effect model. The scale of clinical effect was demonstrated by OR （ Odds Risk） and 95 % confidence interval （ CI ） and the change of blood fat was demonstrated by WMD （ weight mean differ- ence） and 95% CI. The publish bias was assessed by funnel plot and the clinical effect was demonstrated by valid and invalid. Results The OR of clinical effect was 6.45 and its 95%CI was 4.54 -9.15;The WMDs of TC,LDL - C,HDL -C,TG were - 1.03,-0.89,0.13,-0.4 respectively and their 95%CIwere -1.31- -0.75,-1.15 - 0. 63 ,0. 02 - 0. 24 and -0.58 -0.21 respectively. Conclusion This study indicated that atorvastatin had good effect on UAP, and it can effectively improve the disorder of blood fat by decreasing the level of TC, LDL - C, TG and increasing the level of HDL - C in blood. In addition, atorvastatin can significantly decrease the incidence of cardiovascular events.
