摘要
目的:探讨甲泼尼龙联合高渗盐水雾化治疗毛细支气管炎的疗效。方法:将130例患儿随机分为对照组和治疗组各65例。对照组给予改善通气、氧疗、抗感染、化痰、止咳、镇静、吸入沙丁胺醇平喘、补液等常规综合治疗;治疗组在对照组基础上,给予静脉滴注甲泼尼龙1～2 mg/kg,2～3次/d,连用3 d,3 d后开始减量,并予高渗盐水(生理盐水2.5 mL+10%氯化钠1.5 mL)加压雾化吸入,3～6次/d,疗程3～5 d。结果:治疗后治疗组在喘憋和呼吸困难消失时间、咳嗽消失时间、哮鸣音和湿啰音消失时间方面均优于对照组(P<0.05),治疗组总有效率96.9%,对照组总有效率83.1%,两组比较差异有统计学意义(P<0.05),且未发现与用药相关的明显不良反应。结论:甲泼尼龙联合高渗盐水雾化治疗毛细支气管炎安全有效。
To investigate the clinical effect of bronchiolitis treatment by spraying a combination of methylprednisolone and hypertonic saline.Methods： One hundred and thirty patients with bronchiolitis were randomly divided into two groups： the control group of 65 patients were treated with conventional combined therapy,the treatment group of 65 cases were received conventional treatment and methylprednisolone（1~2 mg/kg,2~3 times/d for 3 days,3 days after reduction） combined hypertonic saline（normal saline 2.5 mL ＋10% sodium chloride 1.5 mL） spray.Results： Relieving wheezing,breathing difficulty,reduced cough time,and auscultatory wheezing disappearing time were shorter in the treatment group compared to the control group（P〈0.05）.The total effective rate was 96.9% in the treatment and 83.1% in the control group（P〈0.05）.Conclusions： Hypertonic saline sprayed combined methylprednisolone in the treatment of bronchiolitis is safe and effective.
