摘要
目的对急腹症的腹腔镜手术和开腹手术的成本效益进行评价。方法选择本院2010年4月至2011年4月间有手术探查指征的急腹症患者分为腹腔镜组(132例)和开腹组(109例)。比较两组的住院时间、误工时间、住院费用及总费用。结果腹腔镜组的住院时间及误工时间明显低于开腹组(P<0.05),而住院费用除急性阑尾炎外两者无显著性差异(P>0.05),总费用急性阑尾炎患者腹腔镜组仍高于开腹组(P<0.05),但溃疡病穿孔患者腹腔镜组反低于开腹组(P<0.05)。结论急腹症腹腔镜手术和传统开腹手术相比,随着疾病严重程度的提高,在总的经济负担上逐渐体现优势。
Objective To analyze the cost of laparoscopy and laparotomy in the treatment of acute abdomen. Methods From April 2010 to April 2011, 241 patients with acute abdomen were operated by laparoscopy （132 cases）, or by laparotomy（109 cases）. Patients＂ hospital stay, time tak- en for the patients to return to work, hospital charges and total costs were compared between the two groups. Results The mean hospital stay and the mean time taken for the patients to return to work were statistically shorter in the laparoscopy group than those in the laparotomy group. Except a- cute appendicitis, the mean hospital charges and the total costs in the two groups were similar （no significant difference）. While the mean total costs for the patients with acute appendicitis were higher in the laparoscopy group than those in the laparotomy group（P〈O.05）, but total costs of the patients with ulcerative perforation were shorter in the laparoscopy group than those in the laparotomy group （P〈0.05）. Conclusion With the improvement of illness severity, the costs of laparoscopy operation in the treatment of acute abdomen have advantage than that of laparotomy.
