腹腔镜治疗上尿路结石58例临床观察被引量：2

摘要目的探讨腹腔镜治疗上尿路结石的疗效及安全性。方法2008年1月至2011年2月我科采用腹腔镜治疗58例肾盂和输尿管上段结石患者,其中肾盂结石22例(A组),输尿管上段结石20例(B组),肾盂伴输尿管上段结石16例(C组)。回顾性分析三组患者的手术时间、术中出血量、术后住院时间、术后引流管拔除时间、术后肠道恢复时间和并发症等指标。结果58例患者全部顺利完成手术,A、B、C三组的手术时间分别为(134.6±33.2)min、(82.5±20.2)min和(142.3±37.1)min,其中B组明显短于A组或C组(P<0.05);术中出血量分别为(39.7±30.1)ml、(30.9±10.2)ml和(38.2±33.5)ml,三组差异无统计学意义(P>0.05);术后引流管拔除时间分别为(3.1±0.8)d、(2.9±1.2)d和(3.3±0.7)d,三组差异无统计学意义(P>0.05)。三组患者术后均无严重并发症,随访4～27个月,腹部疼痛和血尿消失,复查静脉尿路造影未见上尿路结石,肾盂和输尿管积液于6个月内均消失,未发现肠粘连等并发症。结论腹腔镜治疗肾盂和上段输尿管结石疗效显著,安全可行,是上尿路结石首选的外科治疗方案。
