摘要
目的 对比分析MRI平扫、增强扫描及MRA对脑发育性静脉异常 (developmentalvenousanomaly ,DVA)的显示价值 ,以期提高MRI对脑DVA的检出率及诊断正确率。材料与方法 回顾分析 12例脑DVA的MRI平扫、增强扫描及其中 4例的MRA表现 ,分别评价其对DVA的引流静脉、髓静脉的显示价值。结果 12例中发现 14个DVA。MR平扫 8个DVA可见引流静脉显影 ,5个DVA扩张髓静脉区有异常信号但不具特征性 ;MRI增强扫描见 12例 14个DVA的引流静脉及扩张的髓静脉均明显强化并呈特征性表现。本组增强MRI扫描显示率达 10 0 % ;MRA仅发现 2例较粗大引流静脉显影 ,扩张髓静脉均未见显影。结论 MRI平扫对于DVA的显示率较低且不具特征性 ,其中对于引流静脉的显示优于髓静脉 ,可作为DVA的筛选方法。MRI增强扫描是诊断DVA最准确、可靠的方法。MRA对于DVA的诊断价值不大。
Objective To improve the sensitivity and specificity of MRI in detecting cerebral developmental venous anomaly (DVA) by comparing pre and post contrasted MR images and MRA.Materials and Methods The pre and post contrasted MRI features in 12 patients with 14 DVAs, and MRA features in 4 of the 12 patients, were retrospectively analyzed. An evaluation of displaying the draining veins and medullary veins with each technique was made.Results On pre contrasted MRI, the draining veins of 8 DVAs and abnormal signal, but without characteristic, in the dilated medullary areas of 5 DVAs were demonstrated. Post contrasted MRI displayed the markedly enhanced draining veins and medullary veins in all 14 DVAs, which were the characteristic sign carrying 100% sensitivity. MRA revealed dilated draining veins only in 2 cases and no medullary veins were shown.Conclusion Pre contrasted MRI displays the draining veins better than the medullary veins, its sensitivity of detecting DVAs is low and its findings are non specific, but it can be served as a screening measures. Contrasted MRI is the most accurate and reliable method in diagnosing DVAs, while MRA has little value in detecting DVAs.
