Transcallosal-interfornix approach to remove the tumors of the third ventricle in children

摘要 目的 介绍并探讨一种切除三脑室肿瘤的手术入路。方法 经胼胝体 穹窿间入路切除三脑室肿瘤 91例。结果 显微镜下全切除 41例 ,近全切除 41例 ,大部切除 9例 ,死亡 1例。结论该手术入路可以切除三脑室前、中、后部的肿瘤 ,术野暴露清楚 ,不易造成穹窿、丘脑、大脑内静脉、丘纹静脉的损伤 。 Objective To introduce a transcallosum interfornix approach for removing the tumors of the third ventricle in children.Methods 91 pediatric patients(ages ranged from 2 to 17 years)with third ventricle tumors were operated on via the approach of transcallosum interfornix.Results Under the operative microscope, the tumors were totally removed in 41 cases, subtotally in 41 cases,and partially in 9 cases.Conclusions The tumors which located at the anterior, middle, or posterior portion of the third ventricle can be removed via this approach. This approach provided a quite large operative field, and the septum pellucidum, the fornix, the inter cerebral vein, and the thalamus can be preserved intact. The postoperative complications were rare.
