摘要
目的探讨老年急性假性肠梗阻患者发生医院感染的特点及危险因素。方法回顾性分析395例急性假性肠梗阻患者的病历资料,对其医院感染的特点及危险因素进行总结。结果老年组154例患者发生医院感染45例,感染率为29.2%,非老年组241例,发生医院感染17例,感染率为7.1%,老年组的医院感染率明显高于非老年组(P<0.05);感染部位主要为呼吸道、消化道、泌尿道等;医院感染的发生危险因素与患者年龄、住院天数、合并基础疾病、侵入性操作、使用抗菌药物有关;感染病原菌以革兰阴性菌为主,占62.9%。结论老年急性假性肠梗阻患者医院感染的发生率高,医务人员应高度重视,控制危险,降低医院感染率。
OBJECTIVE To study the characteristics of nosocomial infections in aged patients with acute intestinal pseudo-obstruction and risk factors. METHODS A retrospective study of the clinical data of nosocomial infections in 395 inpatients with acute intestinal pseudo-obstruction was conducted, the characteristics and the risk factors were summarized. RESULTS Nosocomial infections occurred in 45 of 154 aged patients with the incidence of 29.2%, and nosocomial infections occurred in 17 of 241 patients of the younger age-group with the incidence of 7.1 %, the incidence of nosocomial infections in aged patients was higher than in younger aged group（P〈0.05）; the predominant infected sites were the respiratory tract, gastrointestinal tract, and urinary tract; the age of the patient, hospitalization duration, underlying disease,invasive operations, and use of antibiotics were the risk factors for nosocomial infections; gram-negative bacteria were predominant pathogens causing infections, accounting for 62.9%. CONCLUSION The incidence of nosocomial infections in the aged patients with acute intestinal pseudo- obstruction is high, the medical staff need to attach great importance to the control of the risk factors so as to decrease the incidence of nosocomial infections.
