益气温阳中药干预对压力负荷超载诱导的舒张性心力衰竭大鼠血流动力学的影响 被引量:13

Hemodynamic Research of Tonifying Qi and Warming Yang Medicine in Diastolic Heart Failure Rats Induced by Pressure Overload

摘要 目的:观察益气温阳中药坎离颗粒对腹主动脉缩窄诱导的压力过载性舒张性心力衰竭(DHF)大鼠血流动力学的影响。方法:清洁级雄性Wistar大鼠70只,腹主动脉缩窄术后,随机分为假手术组、模型组、中药组、缬沙坦组和中药+缬沙坦联合干预组(简称联合干预组),每组14只。分别于术后1、4、12、32周监测外周血压,并于32周后观察各组大鼠的血流动力学指标、心肌质量和左室组织形态。结果:与假手术组比较,模型组大鼠造模后第1、4、12、32周历时性前肢血压均明显升高,SAP、LVSP、LVEDP、CMI和LVMI显著升高,差异有统计学意义(P<0.05,P<0.01)。与模型组比较,中药组、缬沙坦组、联合干预组术后第4、12、32周前肢血压显著降低,中药组大鼠SAP、LVEDP降低,缬沙坦组、联合干预组SAP、LVSP、LVEDP降低,联合干预组HR、LVMI降低,缬沙坦组的CMI和LVMI降低,差异有统计学意义(P<0.05,P<0.01)。与中药组比较,联合干预组术后第12、32周前肢血压显著降低,差异有统计学意义(P<0.05)。与缬沙坦组比较,中药组降压幅度小,差异有统计学意义(P<0.05),而联合干预组和缬沙坦组相似。与中药组和缬沙坦组比较,联合干预组的LVEDP更低(P<0.05)。结论:益气温阳中药坎离颗粒能显著降低腹主动脉缩窄大鼠的外周血压,改善压力负荷超载诱导的舒张性心力衰竭。 Objective : To observe the hemodynamic effect of "Kanli Granule" on abdominal aortic constriction in diastolic heart failure rats induced by pressure overload. Methods: Seventy male rats were randomized into sham operation group, model group, TCM group, valsartan group and TCM + valsartan group (integrative group) after abdominal aortic constriction surgery, 14 rats in each group. The peripheral blood pressure, hemodynamic index, myocardial mass and left ventricular morphology were observed after 32 weeks. Results: Compared with sham operation group, the synchronic anterior limb blood pressure at the 1st, 4th, 12th and 32nd week in model group were elevated, and the SAP, LVSP, LVEDP, CMI and LVMI were increased markedly, with significant difference between the two groups ( P 〈 0.05 ,P 〈 0.01 ). Compared with model group, the anterior limb blood pressure at the 4th, 12th and 32nd week in TCM group, valsartan group and integrative group were dropped markedly; the SAP and LVEDP were decreased in TCM group, and the SAP, LVSP and LVEDP were decreased in integrative group. The HR and LVMI in integrative group and the CMI and LVMI in valsartan group were decreased respectively, with significant difference between them (P 〈 0.05, P 〈 0.01 ). Compared with TCM group, the anterior limb blood pressure at the 32nd week in valsartan group and integrative group were dropped markedly after operation, and the CMI and LVMI in valsartan group were increased, with significant difference between them (P 〈 0.05 ). Compared with TCM group and valsartan group, the LVED in integrative group was decreased (P 〈 0.01 ). Conclusion: "Kanli Granule" can markedly drop the peripheral blood pressure in rats after abdominal aortic constriction surgery, and improve the diastolic heart failure induced by pressure overload.
