摘要
目的:观察临床验方"逐水方"外敷治疗脾虚湿盛型恶性腹水的有效性及安全性。方法:将60例符合脾虚湿盛型恶性腹水患者随机分为对照组与治疗组,对照组予支持治疗,治疗组配合使用"逐水方"外敷治疗,观察两组患者腹水B超、中医证候评分、卡氏评分、免疫指标的改变,及对安全性指标的影响,从而观察"逐水方"的有效性及安全性。结果:治疗组缓解率(CR+PR+SD)78.1%,对照组53.6%;治疗组对于腹胀、食少纳呆、小便短少、神疲乏力四项症状的改善,对卡氏评分的改善以及对外周血自然杀伤因子(NK)细胞的提高,均优于对照组,经统计学处理有显著差异(P<0.05)。对于三大常规、肝肾功能无明显影响。结论:"逐水方"外用治疗脾虚湿盛型恶性腹水疗效优于最佳营养支持治疗,能显著改善临床症状,且无明显不良反应。
Objective：To observe the effect and safety of Zhushui Formula through the clinical research of Zhushui Formula external treatment used in malignant ascites of spleen deficiency and dampness syndrome.Methods：60 patients who met the diagnose of malignant ascites as well as match the spleen deficiency and dampness syndrome were randomly divided into two groups.The control group were given supportive care while the treatment group added Zhushui Formula.The Ultrasonic inspection results,TCM symptoms score,Karnofsky score,immune parameter and security indicators were observed.And then the efficacy and safety were compared.Results：The stable rate（CR ＋ PR ＋ SD）in treatment group was 78.1%,in control group was 53.6%.The treatment group improved the symptom of bloating,anorexia,vomit,fatigue;increased Karnofsky score and Nature killer cells（NK）dictator.The results were statistically significant（P0.05）.For the three conventional tests,liver and kidney functions had no significant effect.Conclusion：External application of Zhushui Formula can significantly improve the clinical symptoms of malignant ascites of the spleen deficiency and dampness syndrome,and has no significant adverse reactions contrasted with the best nutritional support.
