摘要
目的探讨乌斯他丁(UTI)改善高压电烧伤病人肾脏功能的疗效。方法明确诊断的高压电烧伤病人40例,其中男20例,女20例,平均年龄(34.32±11.62)岁,随机分为常规治疗组(对照组)和UTI药物治疗组(治疗组),每组20例。对照组按国内常规液体复苏方案进行,治疗组除常规补液外,给予UTI(120万U,静点,每日2次)治疗。对两组病人入院时治疗前及治疗后48h行尿素氮(BUN)、肌酐(Scr)监测,同时监测尿量。对两组治疗前后的观测结果进行对比分析,判断治疗效果。结果对照组入院即刻和48h后的BUN和Scr分别是(6.12±1.30)mmol/L,(94.35±13.20) μ mol/L,(7.74±2.34)mmol/L、(102.53±12.23) μ mol L,而治疗组人院即刻和48h后的BUN和Ser分别是(5.93±1.43)mmol/L,(100.33±15.51) μ mol/L,(5.04±2.74)mmol/L、(90.33±14.32)斗mol/L,治疗组48h后,肌酐检测值下降。对照组48h平均尿量为(56.45±14.77)mL,治疗组平均尿量为(70.66±16.79)mL。结论UTI对高压电烧伤病人肾功能有改善作用。
Objective To explore the therapeutic effects of Ulinastatin(UTI) to renal function in High-Voltage Electrical Burns(HEB) patients.Methods Forty HEB patients(men and women were equal) were randomly divided into control group and treatment group,each group twenty peoples,average age 34.32± 11.62 years old.The control group were treated with universal fluid replacement.The treatment group were treated with UTI(1.2 million U,intravenous drip,two times per day),in addition to universal fluid replacement.Then to detect urea nitrogen (BUN),creatinine(Scr),and measure urine volume of both groups patients at admission instantly and 48 hours after treatment.Then to analysis the observation results and judgement therapeutic effect.Results The BUN and Scr values of control group were respectively (6.12 ± 1.30)mmol/L,(94.35 ± 13.20) μmol/L at admission instantly, and were respectively (7.74 ± 2.34)mmol/L,(102.53 ± 12.23) μ mol/L at 48 hours after treatment.The BUN and Scr values of treatment group were respectively (5.93 ± 1.43)mmol/L,(100.33 ± 15.51) μmol/L at admission instantly and were respectively (5.04 ± 2.74)mmol/L,(90.33 ± 14.32) μmol/L at 48 hours after treatment.The Scr values of treatment group decreased at 48 hours after treatment.The average urine volume of control of control group was (56.45 ± 14.77) mL,the treatment group was (70.66 ± 16.79)mL in 48 hours.Conclusion UTI can improve renal function of HEB patients.
