摘要
目的 :探讨腹腔化疗时联合应用善得定对腹腔淋巴化疗的临床效果 ,为具有高淋巴转移倾向的大肠癌患者开展腹腔淋巴化疗提供实验依据。方法 :对23例大肠癌患者行术前卡铂腹腔化疗10例 ,卡铂加善得定诱导腹腔淋巴化疗13例 ,于术中不同时段内分别取门静脉血和癌灶旁淋巴结标本 ,用原子吸收光谱法测定样本铂浓度。结果 :两组间门静脉血铂浓度无差异 ;1～2小时时两组间淋巴结样本铂浓度相比无差异(P>0.05) ,3小时时实验组淋巴结样本铂浓度高于对照组(P<0.05)。结论 :行卡铂腹腔化疗时联合应用善得定可提高癌灶旁淋巴结的铂浓度 ,诱导腹腔淋巴化疗。
Objective:To investigate the clinical application of intraperitoneal chemotherapy combined with sandostatin to induce intraperitoneal lymph chemotherapy and provide experimental foundation for intraperitoneal lymph chemotherapy in the patients with carcinoma of large intestine who have a greater tendency to metastasize to regional lymph nodes.Methods:There were 23 patients with carcinoma of large intestine in our study,in which 10 patients were given carboplatin in intraperitoneal chemotherapy and 13 patients were given carboplatin and sandostatin to induce intraperitoneal lymph chemotherapy.The blood from portal vein and para tumor lymph node samples were collected in different time during operation.The concentration of carboplatin in the blood and the lymph nodes was determined by atomic absorption photometry.Results:There is no difference in concentration of carboplatin in portal vein between two groups.No difference can be seen in the concentration of carboplatin in the lymph nodes between 2 groups 1 2h after the medicine was given (P>0.05).But 3h later,the concentration of carboplatin in the experimental group is higher than that in the contrast group (P<0.05).Conclusions:In intraperitoneal chemotherapy,the concentration of carboplatin in para tumor lymph nodes can be increased by sandostatin and intraperitoneal lymph chemotherapy was induced.
