摘要
本文研究64例NIDDM肾血流动力学情况。41例无蛋白尿组中已见明显异常,表现为肾小球滤过率(GFR)及滤过分值(FF)增高。病程越短,异常出现率及程度越明显。病程<1年亚组中,GFR及FF明显高于对照。有效肾血浆流量(ERPF)亦呈同样趋向。无蛋白尿组GFR及ERPF显著高于有蛋白尿组。出现蛋白尿后,GFR及ERPF即无增高者,且随蛋白尿程度呈下降趋势。GFR与ERPF及FF均呈正相关。FF尚与以往6个月内平均空腹血糖相关。结果表明NIDDM早期或临床肾病前存在肾小球高滤状态。对其机理及影响因素进行了讨论。
Renal bemodynamics was studied in 64 cases of NIDDM. Significant renal hemodynamic abnormalities, presented as increased glomerular nitration rate (GFR) and filtration fraction (FF) were observed in 41 cases of NIDDM without proteinuria. The shorter the duration of diabetes, the higher the incidence and degree of these abnormalities. In the subgroup with duration less than one year, GFR and FF increased significantly. GFR and effective renal plasma flow (ERPF) were significantly higher in NIDDM without proteinuria than in those with proteinuria (P<0.01 and <0.05 respectively). GFR and ERPF levels did not increase in any case of NIDDM with proteinuria. GFR was significantly correlated with ERPF and FF (P<0.01 and P<0.001 respectively). In addition, FF correlated with the mean fasting plasma glucose level during the past six months (P<0.01). The results showed that there is a glomerular hyperfiltration state in the early or preclinical nephropathy stage of NIDDM. The mechanism and factors contributing to its development were discussed.
