摘要
目的分析睾丸扭转的MSCT和MRI表现,探讨MSCT或MRI诊断睾丸扭转的临床价值。方法收集4例经手术、病理证实的睾丸扭转的影像学资料,MSCT及MRI平扫各1例,MSCT及MRI平扫+增强检查各1例。分析其临床及影像学表现,复习睾丸扭转的CT和MRI表现的相关文献。结果 3例睾丸肿大,1例睾丸萎缩。4例睾丸边界均较清楚。CT平扫睾丸密度不均匀增高,1例增强扫描为环形强化;MRI T_1WI序列睾丸内见片状T_1WI高信号,T_2WI低信号区,精索信号不均,1例增强扫描未见明显强化。结论睾丸扭转在MSCT或MRI上有一定特征性,结合临床表现及超声多普勒,能够提高术前诊断率。
Objective To analyze the image findings and diagnostic value of CT and MRI in testicular torsion. Methods The clinical records, multi-slice CT (MSCT) and MRI of four patients with surgically and pathologically proven testicular torsion were retrospectively analyzed. The CT and MRI were performed with ( 1,1 ) and without contrast (2,2) respectively. Results Testicular volume was increased in three patients and decreased in one patient. The testicular margins were well-defined and the spermatic cords were thickened in all patients. On CT, the testicular density was heterogeneously increased with tim enhancement. On MRI, the testes were hyperintense on T1WI and hypointense on T2WI with heterogeneous signal intensities in the spermatic cords. Rim enhancement of the testis was noted in one patient whereas no enhancement was seen in the second patient. Conclusion MSCT and MRI can help to improve the preoperative diagnosis of testicular torsion in correlation with Doppler ultrasound and clinical presentation.
