肝细胞特异性对比剂Gd-EOB-DTPA在肝转移瘤MRI诊断中的应用 被引量:7

摘要 目的探讨肝细胞特异性对比剂Gd-EOB-DTPA在肝转移瘤MRI检出、诊断与鉴别诊断中的价值。方法收集我院2010年6月~2011年10月经临床、影像证实或高度怀疑的肝转移瘤20例患者,选取8例注射Gd-EOB-DTPA行MRI动态增强扫描及肝细胞特异期扫描,其中6例患者1个月内同时进行了螺旋CT增强扫描;12例注射普通对比剂Gd-DTPA,行MRI动态增强扫描,综合分析两项对比剂MRI增强扫描和CT增强扫描成像特征。结果MRI检查20例患者共检测到病灶83个,其中在动脉期、门脉期和平衡期发现病灶78个;8例注射Gd-EOB-DTPA患者在肝细胞特异期发现病灶55个,其中5个病灶在动态期(即常规三期扫描)未显示;6例患者同期行全肝螺旋CT增强扫描发现病灶31个,对比注射Gd-EOB-DTPA增强MRI扫描少发现8个病灶。结论肝细胞特异性对比剂Gd-EOB-DTPA由于具有双期特性对于病灶的数量、分布、边界有更清楚的显示,在MRI动态增强全肝扫描中更有利于肝脏转移瘤的检出和定性诊断。 Objective To investigate the Hepatocyte-specific contrast agent (Gd-EOB-DTPA) in liver metastases MRI de- tected value in the diagnosis and differential diagnosis. Methods Twenty patients were collected from June 2010 to October 2011, which were clinical,imaging confirmed or highly suspected liver metastases. Eight cases were selected to inject Gd- EOB-DTPA in dynamic contrast-enhanced MRI scan and Hepatocyte-specific scan, while 6 cases had spiral CT scan at the same time. 12 cases were inject Gd-DTPA MRI in dynamic contrast-enhanced scan, compare with the two contrast a- gent enhanced MRI and enhanced CT imaging characteristics. Results Eighty-three lesions were detected in 20 cases with MRI :scan, 78 of which is found in the arterial phase, portal phase, and the balance phase. 55 lesions were found in the Hepatocyte-specific phase of 8 cases with injected Gd-EOB-DTPA, which 5 lesions in the dynamic phase (the convention- al three scan) is not displayed. 6 cases with whole liver enhanced spiral CT scan revealed lesions in 31, contrast injection Of GO-EOB~ -DTPA enhanced MRI scans less found 8 lesions. Conclusion The hepatocyte-specific' contrast agent Gd- EOB-DTPA in MRI dynamic enhancement is more conducive to the liver metastases detection and diagnosis.
