苯拉普利与硝苯地平对高血压病自主神经活性的影响比较

摘要 目的 :比较苯拉普利和硝苯地平对自主神经功能的影响。方法 :选择高血压病患者 ,随机采用苯拉普利或硝苯地平治疗两周。治疗前后记录 2 4h动态血压和动态心电图 ,用以比较降压效果和心率变异指标的变化。结果 :苯拉普利组 18例和硝苯地平组 16例获得完整复查结果。每日平均用药量 :苯拉普利为 16 .6mg ,硝苯地平为 45 .0mg。两组治疗后血压降低的幅度、降压显效率和总有效率均无显著差别。硝苯地平组治疗后LF、HF、SDNN和 pNN5 0降低 ,LF/HF比值明显升高 ,苯拉普利治疗后上述指标的变化与硝苯地平组截然相反。结论 :苯拉普利和硝苯地平在降压作用相当的情况下 ,硝苯地平引起心迷走神经兴奋性显著降低 。 Objective:The purpose of this study is to compare the effects of nifedipine and benazepril on cardiac autonomic nervous activities in patients with essential hypertension. Methods:Hypertensive patients were randomly divided into two groups. One group was treated with nifedipine, the other with benazepril. Antihypertensive effects and cardiac autonomic nervous activities were measured with ambulatory blood pressure monitoring and 24-hour Holter recordings before and after two-week treatment. Results:Sixteen patients in nifedipine group and 18 in benazepril group finished the test. Mean doses of nifedipine and benazepril were 45.0mg/d and 16.6mg/d respectively. After 2 weeks, significant antihypertensive effects were comparable between 2 groups. The mean values of LF, HF, SDNN and pNN50 calculated from 24-hour Holter recordings were significantly reduced, and LF/HF was increased in nifedipine group. The changes of above parameters after benazepril treament were quite the contrary. Conclusion:At the similar antihypertensive efficacy, nifedipine decreased parasympathetic activity, however, benazepril increased parasympathetic activity.
