腺苷脱氨酶、结核抗体及DNA联合检测在结核性积液的临床诊断价值 被引量:6

Clinical Diagnosis Value of Adenosine Deaminase,Mycobacterium Tuberculosis Antibody and DNA Combined Detection in Tuberculous Effusion

摘要 目的:探讨腺苷脱氨酶(ADA)、结核抗体(TB-Ab)、TB-DNA联合检测在结核性积液的临床诊断价值。方法:选择87例胸腔积液进行分析,其中结核组38例,非结核组49例,比较3种检测方法的检测情况及单独检测和联合检测的方法学指标。结果:两组别3种检测方法的阳性率经卡方检验分析,有统计学差异(P<0.05),在结核组中3种检测方法阳性率经卡方检验分析比较,有统计学差异(P<0.05),3种检测方法以TB-DNA阳性率最高,3种方法联合检测灵敏度为92.1%,特异度以TB-DNA最高为97.9%。结论:3种方法联合检测可提高灵敏度,提高结核杆菌的检出率,方法学指标综合分析,以TB-DNA检测为优。 Objective: To explore the diagnosis value of combined detection of adenosine deaminase(ADA),mycobacterium tuberculosis antibody and DNA(TB-Ab and TB-DNA) in tuberculous effusion.Methods: Eighty-seven samples of body cavity effusions from tuberculosis patients(n=38) and non tuberculosis patients(n=49) were abstracted for ADA,TB-Ab and TB-DNA detections.Results: There were significant differences between the two groups in all the three indices by chi-square test(P0.05),and among the three indices in the positive rate(P0.05).Positive rate of TB-DNA was the highest,specificity of TB-DNA was the highest as 97.9%,and the sensitivity of combined detection of the three indices sensitivity was increased to 92.1%.Conclusion: Three indices combined detection can improve the sensitivity and detection rate of mycobacterium tuberculosis,and TB-DNA testing is the best in the three indices.
