摘要
文章从伦理学视角回顾并分析改革开放以来我国公立医院改革政策变迁的五个阶段,阐释每一阶段改革政策的制定背景并对政策执行效果进行评价。在此基础上分析微观利益诱导型的公立医院改革政策的历史局限性,提出公立医院管理体制的改革不能单纯实行诱致性制度变迁,而应是诱致性制度变迁和强制性制度变迁的结合,既要充分发挥自发性改革和基层医疗单位的主动性和创造性,更要坚持中央政府自上而下的领导、组织和协调,对新一轮医疗改革的伦理原则与政策取向进行了探讨。
This article reviews and analyzes five stages of public hospital reform policy changes in our country since the reform and opening from an ethical perspective, explains the policy-making background of each stage of reform and evalu- ate the effect of policy implementation. On the basis, this article analyzes the historical limitations of public hospital reform policies of microcosmic benefit induction; proposes the reforms of public hospital management system can not simply imple- ment the induced institutional changes, moreover should combine the induced institutional change and mandatory institu- tional change, not only should play a spontaneous reform and initiative and creativity of grassroots units, but also should in- sist the top-down leadership, organization and coordination of central government. The ethical principles and policy orien- tation of the new round of medical reform are discussed in this article.
