摘要
目的 :探讨手术方式对外阴恶性肿瘤预后的影响。方法 :对 4 2例经手术治疗且随访 5年以上的外阴恶性肿瘤病人进行回顾性分析。结果 :5、1 0、1 5年生存率分别为 4 7 6%、38 9%、33 3% ,Ⅱ期病人远期生存率显著高于Ⅲ期、Ⅳ期 ,腹股沟淋巴结转移者远期生存率明显低于无转移者 ,腹股沟淋巴结清除对Ⅱ期病人远期生存率影响不明显而对Ⅲ期病人影响明显。结论 :临床分期及腹股沟淋巴结转移对预后影响明显。Ⅱ期病人应选择手术范围较小的手术方式 。
Purpose:To study the influence of operative treatment modality on prognosis of vulval malignancies. Methods:To retrospectively analyse 42 cases with vuval malignancies treated by operation and followed up over 5 years. Results:The survival rates at 5、10、15 years were 47.6%、 38.9%、 33.3%, respectively. The long term survival rates of patients with stage Ⅱ and no metastasis of inguinal lymphnodes were much higher than those patients with Ⅲ,Ⅳ and metastasis of inguinal lymphnodes. The influence of inguinal lymphnodes resection was not obvious on long term survival rates for patients of stage Ⅱ, but marked for stage Ⅲ.Conclusions:Clinical stage and metastasis of inguinal lymphnodes were very important factors for prognosis, extensive vulval resection and inguinal lymphonodes dissection was optimal surgery for patients of stage Ⅲ, but the extent of resection should be reduced for stage Ⅱ.
