摘要 目的 探讨血清肿瘤坏死因子 α(TNF α)在老年痴呆临床前期人群中的作用。  方法  采用RIA法对 5 1例老年性痴呆 (AD)高危人群及 65例脑血栓形成后遗症患者的血清TNF α水平进行了检测。  结果 显示 :AD轻异组 ( 4 6例 )血清TNF α( 5 46 5 8± 110 5 8)ng·L-1、脑血管性痴呆 (VD)轻异组 ( 4 7例 ) ( 5 5 1 78± 83 0 3 )ng·L-1均高于健康组 ( 3 5 4 0 3± 44 3 4 )ng·L-1,具有非常显著差异 (P <0 0 0 1)。AD高危人群中有脑萎缩倾向者血清TNF α水平高于无脑萎缩者 (P <0 0 5 )。全血TNF α水平有随脑智能减退而逐渐增高趋向。  结论  提示血清TNF α异常表达参与它们慢性脑病变的发生、发展 ,动态观察血清TNF α水平可为早期诊治提供信息。 Objective To study the effects of serum tumor necrosis factor α (TNF α) in preclinical aged of dementia. Methods The level of TNF α in serum was measured by Radioimmunoassay(RIA). The samples collected from 51 cases of preclinical Alzheimer’s disease (AD) and 52 cases of preclinical vascular dementia (PVD) in hospital. Results The level of serum TNF α was (546 58±110 58)ng·L -1 in PAD with slight abnormal group( n =46) and (551 78±83 03)ng·L -1 in VD with slight abnormal group( n =47) were all significantly higher than control group(354 03±44 34)ng·L -1 ( n =13)( P <0 01).The TNF α level in AD with slight abnormal cerebral atrophy group was higher than non atrophy group( P <0 05) and VD with slight abnormal may related to cerebral infarction size( P <0 01). Serum TNF α gene express was increased by cognitive function decline. Conclusions The results suggested that an immunological response mediated by serum TNF α is present in chronic cerebral pathogenesis of both PAD and PVD. Observations of serum TNF α level was useful to dementia early diagnosis and treatments.
